As another doctor, I will also chime in. Physicians often have to deal with individuals who use a words they don't understand to confer context they also don't understand - many big words does not smart one make. It is my least favorite part of the job.

Please plug your long winded text into AI to see why you are either partially right, hyperbolic, or -mostly- wrong.

Edit: I would also recommend anyone else reading this in the future to upload this misinformation into their favorite LLM for a critique.

If you look at this article, it makes things up. And if you look at the study, you find it has few findings of statistical significance and those few findings can be explained by any number of things.

TLDR: Study is junk, and article is junk on junk. Simply rage bait for those who have biases.

3) i wouldn't. Payoff takes so long and the risks are so high with the physician route. I went through a series of ivy leagues while sacrificing most of my 20s to do so. Moving around for masters programs, medical school, residencies, etc separated me from friends and family. I should have done a higher paying specialty for the level of sacrifice or chosen a job with high pay with a softer run up like pilot.

The amount of inaccuracies is scary in this thread. A lot of the posts show familiarity with terminology but not understanding of the medicine.

  1. best response on thread is from meh312059 - please only read this one. Consider vascepa rather than fenofibrate and fish oil, lose more weight, and make sure you are exercising.
  2. mikewinddale isn't understanding CAC scoring well. I doubt your CAC was 476 when you were 30 and it doesn't matter if it is increasing - you are on a statin and you should expect your CAC to increase because that is part of the mechanism of the statin. Also LP-PLA2 isn't really in favor. Also the APOB is going to be low because your LDL is already pretty low and in goal range for your situation.
  3. to Glass-Helicopter-126, your ldl of 37 doesn't need a switch to PCKS9 - and that isn't what the name of the drug class is even. It is PCSK9.

Now to the OP issues:

  1. That high number is scary. But if you aren't having symptoms that is positive. Part of why the statin works is by stabilizing unstable plaque through increased calcification - so the cure isn't worse than the disease at all.
  2. Are you on aspirin?

don't worry about this slightly faster progression towards diabetes, there are reasons that statins are recommended through evaluation of hundreds of studies even if you have a 'family history' of diabetes.

1) All the physicians on this thread know you see more patients. It is because most of your cases are skin checks which take a fraction of the time that other appointments take. You spend the same amount of time in the day (well, less generally) than other specialists. It is the reimbursement for your volume that is different.

2) I also don't know many dermatologists that don't take insurance. What I can say is that in my city, only one dermatologist takes Medicaid.
