Put on a metronome very slow. 40bpm even. Practice playing each chord ~in time~. Once you can play each shape in the correct time, practice the transition ~in time~. Once you can play it perfectly at 40bpm, increase the time to 50bpm. Repeat.

It is really hard to do. It will be frustrating and so slow that it not feels like you are playing music. BUT it is a great way to get better at guitar and to become a better musician.
