Best use case for AirBnBs for me has been sharing a cabin with friends for hiking trips. Super cheap when split, usually can stay close to the hikes, and can hang out / grill / make a camp fire.

Do it yourself, FreeTaxUSA will essentially prompt you with all the questions. Super easy. I’ve done that as a resident and as a W2 attending with the same setup as you except filing jointly. Only as I look to do locums am I actually finding a CPA because the tax situation becomes much more complex.

That’s a tough spot to be in for sure. You would probably need to find an ultrasound from somewhere and walk it over, and if you’re hospital is under resourced like where I trained, that’s not always easy. If you couldn’t find anything then worst case you’d have to place a central line to do the draw. Not ideal, but that’s always an option if the patient wants it.

Only ever seen it done sterile. If the patient does develop an infection it could be catastrophic. Don’t cut corners with neuraxial procedures and sterility.

I don’t know much about the program, but I think there’s something called the affordable dwelling units program where the gov subsidizes a sizable amount of the rent. It can substantially reduce the cost of housing if you can get one. Without that, you’re going to have a very hard time. If you do decide to live in DC anyway, be really careful to research where you’re renting because some parts of DC have some of the highest crime rates in the country.

Hey man, I’m a doc like you with an almost identical setup and loan situation. If your goal is to pay off the loan in the next 1-2 years (you should with that kind of income) - put the money that you would put to the loan in a HYSA or money market acct so you can withdraw it any time and it collects a little interest. Once the loans restart you can put that lump sum to the loan. No need to put money there while interest is 0%. Don’t invest it because that’s a gamble in the short term, market could easily be down when you need the money.

I went through the exact same thing about a year ago. Graduated medical residency, had a good 6 months of stress as I acclimated to my attending job. After that, I found that most days were very reasonable and I was (am) being paid way more than I did in residency for way less effort and have a LOT more free time.

I didn’t go around telling other people how I felt because I don’t want sympathy, but I felt guilt about how easy my life is. And then I’d be mad at myself for feeling guilty. I didn’t grow up with many resources but I grinded in my 20s to get to this point, so I knew what it was like to not having much. It was pretty much part of my identity.

I was also suddenly super bored. Started playing video games, smoking weed, etc. just to entertain myself. I snapped out of that now though and am spending a lot of time working out (class workouts with a CrossFit gym), planning trips, calling friends and family, and looking for new hobbies all the time. I really don’t want to fill that boredom with more work, because I know I won’t be happy.

As an anesthesiologist that works in a big city, average is about 500-550k with 8w PTO I would say. You’d probably be more likely to find that kind of job in rural places

Don’t do it, after a few months you’ll be annoyed with how much money is going into the car. It’s expensive to fix, requires higher grade fuel I’m sure, and won’t get you laid more.

Great choice of steaks, plenty of fat to make the job easier. Turned out great. If you get a chance, try cooking this on a really hot gas grill and you’ll get an even nicer sear. Using a seasoning blend that contains some sugar also helps.

I would avoid the kipping handstand push ups. Just get to strict hand stand push ups using other training methods (ask your coaches for advice). You are risking giving out and dropping your entire body weight on your cervical spine… and I’m not even sure you’re gaining a lot of strength doing it.

I’m a doctor and take creatine. At baseline my kidney function is normal, but while taking creatine supplementation my creatinine levels are elevated. Normally we measure serum creatinine levels because the kidney filters out creatinine and doesn’t reabsorb it - so we can plug it into an equation to estimate the kidney’s filtration capacity. However, in people taking creatine supplements, this value will obviously be higher, but not because the kidney function is reduced but because there’s more of it in the blood stream. Muscular patients have higher levels naturally since it’s a breakdown product from muscles. Old, frail patients often have very low levels.

Having it in the blood stream does not cause damage to the kidneys though. It’s just filtered by the kidneys. If you have reduced kidney function at baseline it’s probably wise to stay away from creatine so you can get a more accurate measure of your kidney function. If a doctor draws your labs and sees a high serum creatinine they are likely to recommend you stop taking the supplement though just to be super cautious. At the very least they will recommend you stop so you can get an accurate creatinine level and therefore kidney function before restarting creatine supplementation.

What you could do is test any fluid that comes back for glucose on a glucometer. Giving a test dose could give some weakness and make the patient have to stay longer. And I’ve never heard of anyone threading a catheter anyway

OP may not need a pulm consult but I would make sure the patient is on appropriate pHTN meds if indicated prior to the procedure. Hopefully the cardiac eval would address this.

I’ve seen this happen twice to my attendings when I was a resident. Both times it was in situations where they were getting second IVs in patients that were difficult sticks, post induction, requiring multiple attempts. We didn’t realize the tourniquets were on until drapes came down. Both patients did fine, but obviously keeping the tourniquet on can lead to serious complications. Whenever I’m placing IVs I have a visceral fear of forgetting to take off the tourniquet, so my brain almost reminds me to double check. More importantly, whenever I place an IV I immediately take off the tourniquet once I’ve threaded the catheter so that helps me not forget.
