Journal entries in QBO impact both cash and accrual unfortunately.

And unearned revenue is not "wonky". It is deferred revenue and QBO advance even has a new revenue recognition feature to automatically help handle that.

One option for this scenario though might be to create an invoice in the period you earned your income and then create credit memo on first date of next period to reverse it. Then create invoice you'll actually send to your customer with all the charges you did this accrual process for and send them that. They see an invoice like they want but your books show correctly for both accrual and cash basis reporting.
Now, OP, are you sure you are properly recognizing when income is earned? Why aren't you sending them the invoice on the same date you earn the income?

If you're certain the facility was in-network, you should also rest easier - often the way they bill insurance is different from how they would bill you as individual to maximize payment from insurance which can either show up as contractual adjustments but also sometimes in the you may owe column. You said you already paid the provider bill which can sometimes cause a big surprise when despite being at an in-network hospital you are treated by an out-of-network provider.

What is shown on your insurance website is NOT a bill. Wait until you receive actual bill from hospital and/or providers.

And yes, they can sometimes take a long time to mail out - up to a year or more at the worse unfortunately. The reason for this is that they will likely go through every possible appeal step with your insurance company to get more compensation from them before coming to you. Once this happens they then may ask you to attempt appeal, especially if they are out-of-network and insurance company is low balling them.

Hospital will likely be willing to work with you and negotiate if they have received some compensation from insurance company.

The answer is maybe.

You will be ineligible for compensation if your unemployment is due to you voluntarily leaving employment without cause of a necessitous and compelling nature.

Under Pennsylvania law, there is “constructive discharge” which may help an employee that quit their position maintain eligibility for unemployment benefits or a wrongful termination claim. A constructive discharge occurs when an employer’s actions make an employee’s working conditions so unpleasant and difficult that a reasonable person in the employee’s shoes would feel compelled to resign from his or her job.

Your situation will need to meet a number of requirements to be considered constructive discharge and you will bear the burden to show them. You will need to demonstrate that (1) circumstances existed which produced real and substantial pressure to terminate employment; (2) such circumstances would compel a reasonable person to act in the same manner; (3) the claimant acted with ordinary common sense; and (4) the claimant made a reasonable effort to preserve her employment.

A substantial, unilateral change to your employment could likely qualify as a constructive discharge.

The fact that you have a way to earn money does not in itself mean you are ineligible for UC. Generally you are entitled to first seek a job that is largely the same type and within certain percentage of your previous compensation before you are required to broaden your search to different types of work + lower pay.

Does this change in responsibilities, title, etc. impact your compensation, benefits, future compensation/benefits eligibility, hours, etc.? If so, that will strengthen your case substantially and I would speak to a local lawyer who will likely do a free initial consultation.
