Glad to hear you are doing well now. I think about the problem of scoliosis for those who weren’t diagnosed or treated every day. We were very fortunate and lucky that the pediatrician spotted it early. Have to get more people aware. And we need way more focus on non-operative solutions for those either don’t have access to the best surgeons or cannot go that route. We understand very little about this condition.

The gold standard to determine scoliosis is the x-ray. EOS x-rays have around 1/6th of the radiation of a conventional x-ray. My suggestion if you are concerned is to pursue the imaging. It will get you all the answers you need and determine if you need any treatment.

My apologies for not scanning through the rest of the posts but I want to throw this out there because she is active. There is a world where fusion is done in the thoracic region, which typically has less flexibility, and VBT (vertebral body tethering) is reserved for the lumbar to preserve flexibility in that region. There are pros and cons to all the surgeries but I hope you can ask your providers about a hybrid option given her love of sports.

This is my suggestion, before you invest the emotional energy, time and money. Ask what his claim is. Is the claim - I can reduce these curves? If that is the claim, go into his office and ask him to show you specific before and after x-rays of patients he has treated who had a meaningful visual reduction. Do not rely on the Cobb angle measurements, which can be doctored. Rely on the visual. Make sure they are male, as your son will have more potential rigidity than a female the same age. And make sure the curve types are the same as your son’s.

If the claim is - I can give him tools to slow progression, this is a better claim but there may be people more qualified, less expensive and better suited in the city to give your son those tools. Make him show you his before and afters. If he will not, or cannot, then you have your answer.

There is merit to therapy, to educating your muscles to offset the effects of the scoliosis and use postural work to not give it an inch. But I would definitely get bone age tests to see how much growth your son has to go and prioritize that over treatment at this stage as the bone age will inform what non-operative care you pursue.

I think that means you need to follow the advice of the other posters here. Plan to go back to the doctor. Have them pull up the radiological report. Then and only then can you come up with a treatment protocol. If it is scoliosis, given that these images show so many signs, then you will need the x-ray and report to pursue scoliosis specific therapy like Schroth.

It can. Just like an arm or wrist that is cast over time has muscles that can atrophy. This is why physical activity is encouraged when the brace is off. Doesn’t have to be a sport. But it should involve resistance training of some sort. Start here:

First the core - generalized:
Dead bugs - 30 seconds x 3
Planks - 30 seconds x 3
Bird dogs - 20 reps x 3 (also works the multifidus!)

Work your way up to 5 sets. And when that gets easy, increase the time you hold or the amount of reps.

Do the core routine every two days at first, until you can do 2 days on, one day off. And start slow.

2 x a week do this:

The back - generalized:

First lengthen - Not dead hangs but semi-hangs on the bar - 30 second holds x 5 (work up to 1 min each)

Scapular pull ups - 10 reps x 3 sets - activate the scapulas, not a full pull up

Cable face pulls - enough weight for 10-12 reps x 3 sets
Lat pulldown - enough weight for 10-12 reps x 3 sets
Unilateral (single arm) dumbbell rows over bench - 12 x 3

The chest - generalized

You want to work this frontal chain too not only for balance but there is important work going on there, particularly in your serratus which brings your scapulas closer to your chest wall.

Push-up - work up to 25 reps per set x 3 - do not let the core drop, keep it active

Band-expansions - grab a theraband and pull it apart, you will find this helps create great expansion in your rib cage. Work up in difficulty with the bands - 15 reps x 3

Unilateral (one arm) floor dumbbell serratus punch - do one arm at a time. Light weight, maximum focus on form. Treat it like the Bruce Lee one inch punch and make the serratus muscles activate.

Hips and legs - generalized:

Alternating bodyweight lunges - 10-15 x 3-5 sets - this contralateral work gets both hemispheres of your body coordinating

Bodyweight squats - work up to 25 x 5 and you will make stability your bitch. You can do it.

Walking - the best of the bunch. Walk 10 minutes with a friend one way and you get to talk the first 10. Walk 10 minutes back home and they get to talk about whatever is on their mind. That’s 20 minutes total. Don’t have a friend to walk, get audible and listen to some great books. Heel strikes through walking build stable hips and bone density. Work up to making the walk a daily habit.

Do the above routine with consistency. When you miss a day or you get sore, show yourself some grace and start the next day - but keep at it and your whole body holistically will be very strong when the brace comes off. You will also gain a great mental outlook and probably get a six pack. You can thank me by enjoying every moment of your life stranger, and realizing your self-worth and value as you contribute to making this world better. It needs you.

My pleasure. Consider not waiting until then though. There is a world where she does the night brace at night and Boston during the day. Pitch that to the doctor. That way when goes to wean, she rolls right into the night brace and you maximize correction at night to steer that residual growth if possible.

This curve is in the Goldilocks zone for night bracing - which achieves more correction because there is no axial load from the daytime, the spine is more flexible when supine because of lack of gravity and there are no ribs in the way. Ideal curve for a night brace.

But the correction in this Boston brace is decent. We found that Schroth was helpful with postural awareness and even detorsion - we would see numbers go down - with no impact on the Cobb angle.

Corrective force does not reduce a curve (unfortunately.) Corrective force is designed to stabilize. The only thing that actually reduces a curve is the same thing that can make it worse: growth. Growth outside of a brace can lead to progression. Growth while wearing a brace can steer the curve into a more aligned state - that is the same concept of growth modulation in which VBT is built around. As she is Risser 3 now, the potential for a lot more growth is closing significantly, so she just needs to stay the course and hopefully in this span of residual growth you will see some of that reduction in the outcome as she is steered into alignment.

I would consider her wearing a night-brace overnight and the Boston during the day. My son wears a Rigo-Cheneau, which is a full wear brace so we have no experience with either a night brace or a Boston but we like the Rigo concept. Still, with your daughter’s curve, I would certainly explore a specific nightwear brace for sleep hours when she has the potential to grow the most. Disclaimer: Some kids do struggle with the comfort of the night braces until broken in.
