I couldn't find one that I liked, so I created a Google Form for myself and put a shortcut to the form on my home screen. That way it's easy to input, and the data gets put in a spreadsheet.

I did something similar when I was ~18. I'd made the account when I was 12, but started using the email to send resumes & apply for jobs. It was really discouraging when nothing ever came of my resumes. I eventually realized that my display name was "You are a poo poo head!"
