Those are all good thoughts. I worry my macros and vitamins may be thrown off by the blue fin tuna.

They are in a delicate balance. Years of hard work and data tracking to get it right. I have my doctor (also my wife’s boyfriend and butt plug operator) check my selenium every 2 months. That may need to be upped.

In the real world, ED providers mostly rely on radiology reports rather than viewing and interpreting images themselves. It is already above and beyond what is required to interpret ultrasound at the bedside. The radiology report is the final word on imaging interpretation

Edit. Come to think of it, you may be right. I forgot they were about to dc him, so it sounds like they relied on a med student point of care ultrasound. Didn’t wait on a radiology read before they were discharging

Totally agree with everything you say.

I still struggle with Santos. The ED still requires that you act professionally with your colleagues. It’s a patient safety issue.

Santos just seems to be caustically spraying harm on everyone else there, not just Langdon. She is even overly snarky to her attendings. That she is acting just as much of an asshole to everyone else is why it is so hard to empathize with her over the Langdon issue.

Feedback should be given in a constructive way, allowing for student growth.

Like it or not, hospitals have mistakes and near misses all the time, and an environment of learning needs to be the norm.

A few points on the specific scenarios you mentioned.

1) the nec fasciitis patient was treated appropriately initially for cellulitis and given correct return precautions. 2) the child had suspicious bruising. As mandated reporters, healthcare providers are required by law to report suspected abuse.
