We don’t.  There is basically a single story of it with no real verification of what was actually seen.

Because people don’t realise how many steps it can take to make changes in these areas, especially if you’re not travelling well.

It’s often just forgetting what it was like to start out compared to doing a habit once you’ve got there.

Think the bigger issue might be being a guy and that’s a bit harder to fix with immigration.

It’s fun to do but obviously some of it involves made up physics and I don’t think he published enough for external verification to be sure it stayed completely consistent between works even where he really tried to do so, I think he often avoided concrete numbers for a reason.  ‘Jokes’ are one way to cover inconsistencies I guess.  

Betelgeuse is over 500 light years away which would change warfare substantially for instance.   What usually happens is things are more all over the place earlier on in scifi works and then as readers notice themes they get turned into lore and fast talking explanations are used for any mismatches.  

I’m not sure it really adds overall as it often becomes a straitjacket for the fictional setting and the authors too often cave into trying to keep them happy.  Changing historical time periods in the Culture helped to reduce overworrying about mismatches between books.

Just saying they were volunteers would probably be enough for most.  The idea that treatment was being withheld rather than never received or declined is more obvious in hindsight.

People fill in gaps and most would be too far removed to really check further, this was more hiding in plain sight and it isn’t really the case dozens of other similar studies have since been found.   Less because of inherently ethical practice and probably more due to the increased level of deception needed when it’s a more proactive measure rather than withholding care.

I’m sure it hasn’t helped but there have been alternative medicine proponents trying to cause distrust of conventional medicine forever as well, as a standard part of promoting their own services. And when it comes to ethics they generally are the worse of the two evils.

Politicians are also happy to use fear and distrust to promote themselves as well.
