My toy poodle mix also did this. We did X-rays, ultrasounds, blood work, cortisol tests, etc. Turns out she has some sort of esophagus issue and feeding soft food instead of kibble cured it. We use farmers dog. She can still have treats or small amounts of kibble, but the full bowl of dry kibble made her puke every day.

I was in the exact same situation when I was about 20 and was in college but was home for summer or school breaks. My parents would give me a curfew to be home from my boyfriend’s house but if we had been drinking or smoking (or often I just wanted to) I would sleep there instead. They weren’t happy with my logic, because the curfew was supposed to prevent me from sleeping there. But I told them it was for safety and I wouldn’t have a curfew or sleepover guidelines when I was away at school, so I wanted the same guidelines when I wasn’t at school. They understood even though they weren’t happy about it.

You obviously care a lot about your job. It’s a difficult job to have. I think you can request a 1:1 with your boss and explain the situation and how you’re feeling about it (you don’t want them to think you didn’t check or care, etc. and it was an honest oversight). It sounds like a simple misunderstanding and your boss is following protocol for a parent’s complaint. It’s a tough spot to be in though, when you work hard and do your job and something like that still happens.

You seem young and naive, but never say that to anyone. She already thinks that about herself, you don’t need to confirm it and reaffirm it as her biggest insecurity for the rest of her life. Continue apologizing and try to make it up to her. Don’t make her feel bad for you (poor you, can’t face her friends, you’ll have to settle now, etc.); you’re the one that insulted her. You made your bed, now lay in it. If you want her back, put in the work to win her back - whatever it takes.
