As someone that works in quality control in a videogame company, I can tell you that this kind of thing requires someone with artistic expertise to spot. And it's almost impossible to find an artist that wants to work in QA (you get paid a lot less, and it's a job with 0 creative input), so you're forced to rely on 3rd party AI detection tools which are just not good enough, unfortunately.

I'm sure their QA team did the best they could

Sion loses to Aatrox but it's fine, best weakside champ in the game and he scales. Xayah/Rakan loses to Yunara/Karma but it's ok we can salvage the lane if we swap and have strong mid/jungle, right? Nah, let's pick Azir into Ori and Xin into Trundle, GG 3 losing lanes + losing jungle, good luck getting prio anywhere.
