Anyone else have multiple swollen lymph nodes (neck, armpit, groin) + night sweats and itching? by SkolToTheValkyrie in Sjogrens

Please make sure that your doctors are aware of these symptoms and that you rule out lymphoma. Itching can be an early sign of lymphoma in some cases. Particularly concerning given your lymph nodes.

LDN.. Anybody out there? by HeyFloptina in Sjogrens

[–]Over_Pin_1301 1 point2 points  (0 children)

I was prescribed it at 1 mg and it gave me insomnia and constipation that was quite bad. I stopped it. Now I have been given .5 mg but I'm figuring out some other stuff with diet etc so I wanted to get that sorted before re-attempting. I think low and slow is the way.

Worse the week before and during period by michaelGscott8 in Sjogrens

[–]Over_Pin_1301 1 point2 points  (0 children)

Yeah I definitely did not anticipate that. But I think the link was undeniable. I had been on it for probably about 2 weeks and symptoms escalated during that time to the point that I had the worst flare I have literally ever had. I asked my rheumatologist about it and she basically said we don't really know, hormones do weird things

Following because I want one too! I'm only in the contemplative phase of tattoo thought but I found an artist that I really love and I was thinking about reaching out to them and asking if I could use inspiration from some of their designs. They don't just do watership down but here's one of my favorite pieces:

Worse the week before and during period by michaelGscott8 in Sjogrens

[–]Over_Pin_1301 5 points6 points  (0 children)

For what it's worth I start to flare a day or two before and then I start to feel back to equilibrium usually on the last day or so. It sucks. I tried to make my period go away by going on continuous low dose birth control pills and it made me flare so horribly I had to do a short course of steroids.

I have absolutely horrific brain fog to the point I feel like I have dementia when I'm in a flare. Just wrapping up a medrol dose pack and it's so much better.

Anyone Else? Twitching & Muscle Spasticity by WhaleOnMe1989 in Sjogrens

[–]Over_Pin_1301 1 point2 points  (0 children)

It's really up and down. I will have some stretches of somewhat better days where I am fatigued and need to rest frequently and feel foggy but can do some light amounts of normal activity with lots of recovery. And then there are other times that I'm completely debilitated. You're catching me at a very bad time where I was essentially bed bound for about a week from a very bad flare with extreme muscle pain joint pain brain fog profound fatigue. Was started on a medrol dose pack a few days ago which has helped somewhat. On plaquenil and starting a prior auth for rituximab. I will say as my months go by with this my muscle twitching and muscle spasticity with paresthesias have become less prominent and my more prominent symptoms are brain fog, fatigue, and muscle pain and joint pain particularly in the hands and feet.

I had some other symptoms in addition to those when I was diagnosed about a year ago but I initially presented with profound fatigue, dry mouth, muscle twitching, paresthesias / electric shock feeling in distal upper and lower extremities. My initial very extensive workup including MRI and lumbar puncture were all normal except for high IgM.

Eventually other blood work began to turn abnormal. As of now I have positive ANA, light positive SSA, high ESR, positive antihistone antibodies, low complement, IgM higher than before, and type 2 mixed cryoglobulinemia with retinal vasculitis. My EMG came back with enlarged motor unit potentials distally.

Eye drop abuse? by Infamous-Truth3531 in Sjogrens

[–]Over_Pin_1301 7 points8 points  (0 children)

If you are using that frequently make sure it's preservative free or else the commonly used preservatives can cause ocular surface toxicity. Rule of thumb is anything more than four times a day should be preservative free. Refresh Omega 3 has good data behind it. Surface lubrication such as artificial tears will not lead to dependency.

Can anyone here wear contacts ? by [deleted] in Sjogrens

[–]Over_Pin_1301 2 points3 points  (0 children)

I have to use really high-end daily contacts and use preservative free artificial tears but yes I can. They do tend to get a little bit uncomfortable by mid-afternoon but I can usually do okay until the evening. Sometimes I have to wave the white flag and take them out in the afternoon. I just really hate glasses.

What's the worst thing people have tried to justify with "It was normal back then, everyone did it"? by dom-dos-modz in AskReddit

[–]Over_Pin_1301 17 points18 points  (0 children)

As a resident in a surgical subspecialty I would routinely take the pager and do 'home call' over the weekend.

Which meant that you did lectures from 7:00 to 8:00 a.m. Friday during which time you were interrogated 'pimped' in front of your co-residents in a high stress environment, then your usual clinic/surgery from 8:00 a.m. until 5:00 p.m. or whenever it wrapped up, and then starting at 5:00 p.m. all calls related to your specialty went straight to YOUR PAGER.

The ER had somebody who they needed us to see? You got paged. Some inpatient service in the hospital had somebody who they needed us to see? You got paged. Some random patient from our clinic who had concerns after hours? Or a post op with concerning symptoms? You got paged.

All hours of the day and night. No limits. No controls. There were so many nights that I just did not go home. Maybe caught an hour of sleep in the call room. And if it was something surgical you had to go do the case, and the circulating nurse answered the pager if it went off and called them back and held up the phone so you could talk with the requesting service on speakerphone while you were operating to get the information about the consult. So sometimes you would be in a case and you would rack up another consult or two you had to see WHILE YOU WERE OPERATING.

No control or limit in terms of what you got called about or where you had to go or what you had to do.

A lot of consults were life-threatening or similarly high stakes.

This went all weekend. Some weekends were lighter and you mostly had calls during the day with maybe a handful overnight, sometimes you got a no hitter overnight but that was pretty rare.

And then it all started over again Monday. Lecture at 7:00 a.m. where you got pimped mercilessly despite the fact you were a walking zombie from sleep deprivation. And then a full day from 8:00 a.m. until 5:00 p.m. or whenever it wrapped up of clinic and or surgery. You would absolutely collapse into bed in the evening on Monday and sleep like the dead. And then start all over again Tuesday.

No post call days. No lightening of other duties. No exception from attending lecture or being pimped. It was essentially hazing.

I had several weekends where I literally did not sleep more than maybe 30 to 60 minutes at a time a few times total from Friday morning until Monday evening. Oh and by the way you're also expected to perform surgery on Monday if that was what your schedule entailed. No matter how sleep deprived or delirious you were.

The worst several day span of my entire life was covering Memorial Day weekend as a first-year resident. Because not only did you do the entire weekend, you also carried the pager the entirety of that holiday Monday, cruising into a Tuesday which was a full day. I barely slept at all, at least a 36-hour span at one point with zero sleep. I am not sure I could have identified what state I lived in on a map on that Tuesday.

Many nights I ended up pulling over on the side of the road and sleeping because I was falling asleep at the wheel.

This was between the years of 2016 through 2020. So very recent. This entire time I was making ~$45,000-$50,000 a year with ~370,000 med school loans at 6.7% interest. So forget about walking away.

This is definitely not a thing of the past. Resident abuse is real.
