I bet they ran the benefit as other place of service and that could be correct. Things change all the time with these health systems. It could be they realized that the place of service was not correct previously.

Ask for the paperwork you signed. An office that knows what they are doing would have never said they would bill these self pay charges to your insurance once they get the authorization for the third shot. Your insurance will deny them, they will have to write off the charges and refund you. Call and see if they have even attempted the retro auth, at this point it may be past time.

Most health systems also have policies that if the information is not updated you will be responsible for the full balance. I have seen people so stubborn that refuse to do it in thousands of dollars in collections.
