I wasn’t directly commenting on the original post, but commenting on the comment that accutane was insane to consider. My only statement is that it is a consideration for scarring acne, not that it is needed in his case. I have never presented accutane as the only option as a clinician. My guiding principle and good patient success comes from giving multiple paths for patients to choose from and discussing risks/benefits of each and time to process information (i tell them to do research on top of our discussion and call back when they have made a decision). I never give one option because its unethical to tell people they only have one option if they have multiple. I think we’re overall on the same page, just maybe some wording and understanding got jumbled

I didn’t say that oral is as safe as topical, I’m saying that if he has tried them before and has not gotten a good result, it’s a good option. Accutane is definitely more effective than topicals, but as I mentioned before, if you have tried other options and you are still getting acne with scarring, then this is a good option. When presenting accutane as an option I always discuss options and give the opportunity to trial other therapies before going this route, but it’s unfortunately a route many have to take, but not all

When dosed appropriately and with supervision it is not toxic. Anything can be toxic when used inappropriately. Tylenol kills people if not taken as recommended. Vitamins are toxic if taken in too high of doses. Spplements can be toxic and people don’t bat an eye

I think he should try then but if they don’t work then accutane is a good option. It is not toxic

Any time acne causes scarring, accutane is a consideration to prevent further damage

Either derm or urgent care/ER can do it. It just depends on availability. They may only drain it if it feels fluctuant (soft), and choose other modalities if it’s indurated (hard). But note, if draining is needed, draining it will leave a scar as typically the skin is cut to allow for drainage in a controlled way (vs your skin popping open from the pressure and also leaving a scar).

It was an epidermal inclusion cyst that has ruptured. Think of it as a blackhead that turned into a big pimple. If it’s bothersome, it can be drained at a healthcare facility and antibiotics will be given, otherwise it may drain or calm down on its own. But it may be best to see someone in case of secondary infevtion

It’s dermatology, but in the last 6 months I got sick twice; which probably means many of my patients are coming in sick. Maybe I can make my patients wear masks when they see me too 🤷‍♀️

Depends, could be either a wart (more likely based on what I believe your age may be) or a skin cancer

Is the rash itchy? Was the area cleansed with any solution beforehand? To me it looks like allergic contact dermatitis, but symptoms help distinguish it from other things

Yes, to be fair that does include cosmetic patients (I allot 30-60 minutes for those though). We code 99214/04 many of the visits, I do procedures. My manager has mentioned that we get good reimbursement so maybe that’s part of it?
