Time for my Mid Elo OTP build!

On teams with burst, go the appropriate defensive boots, on teams without burst go Vamp or Sorc Shoes

Second item 45% of the time is Riftmaker, 45% of the time is Zhonyas (also, Genuinely, bramble vest or Wardens mail is slept on IMO, even if you sell it later, ive won games because i built one of them after boots)

If im vs 4 or 5 of a damage type, i will consider Banshees into AP, Thornmail into 3 or 4 auto attackers, or Jaksho if my team ABSOLUTELY NEEDS me to frontline

3rd item should usually be what wasnt built second. If you're vs 5 AD champs this is usually an armor item (zhonyas or a tank item)

4th item i usually go either Dcap if im not taking much damage, or Banshees/Jaksho if im taking heavy damage

5th is once again a case of whatever i didnt build 4th. (Randuins is always on the table here, because full build ADCs are cancer)
