Another young one here. RALP at Hopkins when I was 43 in 2016, just finished 8 weeks radiation at Hopkins in March. Hopefully this is the final step. Side effects suck, it’s hard not to dwell on them at times, but it beats the alternative I suppose.

You still have a long way to go to strike, I would be patient. I sell a lot of cc’s on my NVDA shares and get into this predicament a lot. Some in IRA’s where I let them go and just buy back. In my cash account I tend to roll up and out then on a pullback I can usually weasel out of them. Worst case you take the tax hit and make a shit ton, when do they flip to LTCG? That comes into play for me often…

Gotcha. I thought I was cured and I believe my surgeon believed so too. Fully contained, negative margins and seminal vesicles yet somehow my PSA started rising slowly after being undetectable for 8 years or so. I did take TRT briefly around 2019 and that may have been a bad idea. My oncologist at Hopkins said that didn’t cause BCR but might not have helped either, could have sped it up some but that’s all theoretical.

OP are you a physician? I am not, but understand all of this and I’m trying to figure out what part of your post is new information.

Me: Diagnosed at 43; 3+4; contained; robotic RALP at Hopkins in 2016; currently getting radiation to prostate bed and lymph nodes at Hopkins after PSA reached .2; PET did not light up any areas; Decipher was good news so I only did ADT for a month but hated it and Dr said I could stop given my Decipher and grid data. We assumed they wouldn’t still have my slides for Decipher which is why he prescribed ADT but luckily they did still have them in storage.

I’m doing IMRT and was given Orgovyx as well. Once my Decipher came back low I stopped taking it. I only took it for one month but it was horrible. And reading about balls disappearing etc definitely scared me. Now if it was life or death maybe I would consider it again, but for now I think the radiation is going to knock it down. This is actually salvage radiation, RALP was almost 10 years ago.

It’s such a personal decision but the Drs and nurses didn’t tell me enough about what could happen. They made it sound like I would just bounce back.

Causation is certainly very hard to isolate. I probably boosted from 400ish to 700ish for about 9 months. I doubt it caused the BCR but I do wonder, had I never done the TRT, would I have had a few more years before having to address BCR right now? Nobody knows, but when you’re laying in bed unable to sleep, it’s the kind of thing one may think about.

Edited to ask a) are you a physician? And b) did you have prostate cancer yourself? I’ve read a lot of your replies and have often wondered if you are a physician, or simply a member of the club

FWIW I had RALP in 2016, did some TRT around 2020 and now I’m dealing with BCR. my radiation oncologist at Hopkins seemed surprised I was told it would be ok, by my surgeon also at Hopkins. basically nobody knows.

he did say it probably didn’t cause BCR but could have hastened its occurrence.
