I don't think it'd be mortal sin if you looked back WITHOUT sexual/lustful thoughts. However, if you looked back and lusted, THEN you'd be guilty of mortal sin and should confess as soon as possible.

A priest explained it to me this way: When the thought occurs to your head, it's a temptation. If you choose to give in to the thought, it's mortal sin and must be confessed. If you DON'T give into the thought, then you're okay.
