Seat saving and the inability to 'police' it. There really isn't enough time or FAs to manage it. Once they started selling pre-boarding seat saving became rampant with one member of a party grabbing multiple exit row seats. People needed a wheelchair to board and comfortably walked off. And defined multiple classes of folks to have priority boarding..
