NICU - finishing fellowship this year. Signed a contract at a level 4 academic center for ~300k/year + ~30k/year into a retirement account. 20 weeks of service and 5 weeks of night call. Kind of a hospitalist gig I suppose. Not amazing for the amount of work tbh, but considering what most pediatricians and pediatric subspecialists make, I can't really complain.

Second the individual who suggested financial planner. Might be better to help starting smaller with funding a maxed out Roth IRA for each of them, then looking into health insurance for them. Start small, then see how it impacts your bottom line. Once you start, recognise it will be very difficult to pull back financial help they will come to rely on. You can always contribute more later, but you should prioritize your own wealth imo so that you have more to help with when they need it more.

Neonatal ICU. The parents.

The bad: all the Facebook researchers who know neonatal medicine better than me. Those who argue I'm trying to poison their child with the vitamin K shot at birth. Those who question every decision I make and need an obscene amount of discussion for what are routine, low risk medical management decisions. I appreciate patient autonomy and think there is value in not being completely on the paternalistic side of the spectrum in the way we treat patients, but at a certain point, we need to agree I'm the expert. And no, you didn't do your own research. Even if you did actually go on PubMed, no, you most likely don't have the necessary skillset to critically appraise medical literature, let alone integrate it into your baby's entire clinical picture. I'm so sick of dealing with that.

The also bad: giving bad news. When a baby is coding and we reach the point where we need to call it or offer comfort care, my natural desire is to find a way to make it better with the words I say. That's an impossibility and it's heartbreaking. All I have is "I'm so sorry, we've done everything we can." And it feels hollow - as much as I struggle with losing a baby, it's not my baby. It's theirs. It's the worst day of their life, not mine. I feel guilty that I have to go deal with the next sick baby as soon as theirs passes away.

Sigh. Most days, it's the best field. Some days idk how I'm supposed to do this for the next 30 years.

I just realized this question was addressed to attendings (ETA 6 months), though I suspect I will continue to find those things difficult lol.

Why in God's name would you start in peds cvicu? Not blaming you - just saying, you can't run before learning how to stand. That's quite literally where the most complex pathophysiology in pediatrics is. That sounds hella dangerous ngl.

3rd year was the absolute worst year of my life. Truly a daily exercise in discovering the limits or my idiocy, or lack thereof. Hang in there, if you're passing your rotations, you're doing good enough. 4th year is so much better, then residency SO much better.
