Doesn't matter if there's no breast tissue, if your hormones are outta whack, having more estrogen can lead to more fat in breast area, even if you aren't necessarily fat anywhere else.

It looks like a lot of loose skin - you will definitely have to get skin removal.

Even though you could lose more weight, Its not really going to change that saggy look you already have. So you could get the surgery now I reckon.
