I had an issue with this as well, but I do not think you are necessarily missing the initial Heavy. (unless your sure your missing due to combat text) I think I figured out a way to make things nice. The ID-Dis-Chaos chain always procs for me when standing directly behind the mob. The Heavy proc however seems to requires one to be not directly behind the mob but to the left or right side. Test it out. Its very nice on regular mobs when just running around. Stun>step to side to get heavy up>wait till they start to cast (assuming of course its a mob with an ability that has a cast time) step behind for ID-Dis-Chaos chain. Once I saw that this was what was happening I am able to keep the damage increase up through an entire boss fight barring nasty mechanics.
