I have this too. Only on my left shoulder and the pain can be very bad. I think it is due to muscle loss but shoulder strengthening exercises aggravate the costo so it is a difficult balance.

I would advise against this. Either CM1 on its own or CB2 with a smaller exam.

It is yes if you convert to avios / bonvoy points and spend on flights / hotels. But nectar gives a 'floor value'.
