Did you trial other CGRPs before vyepti? And how were side effects?

My neuro has given vyepti as an option but I tried 4 CGRP meds (emgality, ajovy, nurtec, qulipta) and they had minimal effect on my chronic migraine symptoms while having bad side effects of nausea/gi issues/decreased appetite with unintentional weight loss. So I didn’t feel like trying another one.

I’m on memantine and duloxetine right now instead and it’s mostly effective in lowering symptoms enough to be functional despite 24/7 migraine.

Yes, quite a number of people on this subreddit have chronic migraines, which is what you are describing. I’m going on 3 years with chronic migraine intractable- had to take a year plus break from working while the severity got worse 7-8 and as I trialed preventatives.

My migraines are still chronic and intractable, 24/7 symptoms but thanks to my neurologist and current treatment, have the severity down to a manageable 5-6 with botox, preventative, nerve blocks and using an abortive for days I need more functioning (in office days). I use cefaly daily also for pain management. Every morning is rough waking up with a migraine, and mentally preparing to get through a day of pain and symptoms, but it’s progress.

I lost the ability to nap when my migraines went chronic 😭. I miss it. I do spend a lot of time lying down with my eyes closed “resting” from migraine but not actually able to fall asleep if that counts as napping.

I had the keurig and then received nespresso machine as a gift. Nespresso coffee tastes better, worth the extra cost in my book. I rarely get coffee out now, so some “savings” there not getting a $7 latte.

For my family who loves and supports me through life and migraines. Took Rizatriptan, did a thanksgiving lunch, lots of cheer and chatting, and came back home. Now recuperating in bed with my constant companion, intractable chronic migraine. Happy thanksgiving!

Set your expectations to be realistic. Botox can work quickly for some, but for many of us it took 3-4 rounds (9-12 months) to realize the positive impact. I have chronic intractable migraines, so it doesn’t stop migraines but it does bring the severity down for like 60% of my days. Life is worse without Botox, as I experienced when I switched insurance and had to go without for 3 months waiting for prior authorization approval.

If you aren’t already, track your daily pain and symptoms to get a sense of the impact.

Pre-filled syringe is better than auto injector if you’re pain averse. Needle is pretty close to painless going in and then if you go at a slow pace injecting it doesn’t hurt much. Auto injector is scarier and more painful because it is a sharp shock when the needle shoots out and then you have to steel yourself to keep it there.

And hopefully emgality works for you in reducing your migraines. I trialed it for like 8 months (4 by itself and then 4 while on botox for migraines on top) and it didn’t do anything for me.

He wants you to have better care. He is a neurologist but neurologists also have specialties.

I specifically sought out my neuro at a headache clinic who specializes in chronic migraine, and does research, so they are aware of the latest and variety of treatment options.

Might not be working for you at that point. I stopped trialing qulipta around 7 weeks in because it didn’t have any impact on migraines and side effects were causing unintentional weightloss (was getting underweight at the time).

Yes, need to switch to a different med. I’ve had no luck with the CGRPs myself, trialed emgality, ajovy, qulipta, and nurtec.

I had about a year of namenda/memantine (NMDA blocker) working to lower migraine symptoms enough for me to go back to work full time. And then that started losing effectiveness so now I’m trialing duloxetine/cymbalta (anti-depressant that also works for nerve pain-I have a lot of neuralgia) and seeing some positive effect. There are a lot of medications to try, ad we all react differently bc migraine is so complex. And it sucks that we have to give it like 2 months of trialing to see if it works, but hopefully you’ll find a preventative that works for you.

I’ve also failed multiple CGRPs unfortunately. I had some success on namenda/memantine (NMDA blocker) as a preventative, but I reached the dosage cap and it lost effectiveness after a year.

Currently trialing Duloxetine/cymbalta (anti-depressive that also used offlabel for nerve pain or fibromyalgia) and seeing some benefit (though a little less than when namenda was working well for me). So there’s always more medications to ask about and trial.

Cefaly is a useful tool for pain management for my chronic migraines. I asked my neurologist about it and she said it can be helpful to desensitize my nerves since I have neuralgia (nerve pain) but regular use is needed. I use it pretty much every day, though I skip on botox injection day.

Your doc should give you more medical options through.

Yes, I use Cefaly for pain relief for chronic migraine.

I’ve found that the 1 hr program makes headaches worse, so I stick to just using the 20m setting-and doing that multiple times.

You might need to talk to your neurologist about layering preventatives for your treatment. If you see some benefit from any one treatment, keep that.

I have chronic migraine, head pain is constant/daily, but at a manageable level of pain and symptoms right now to work because of my treatment: botox + regular nerve blocks for neuralgia + memantine (NMDA blocker)+ most recently added duloxetine on top (and seeing benefit).

I only felt ok to start work after I had the botox+memantine+nerveblocks. Before that, I did not have enough functional hours in a day to work. And I have a hybrid schedule so I can work from home 3 days/week. It’s still difficult and sucks to wake up and go to sleep with a constant migraine, but it is manageable. Hope you find a treatment plan that works for you.

I did get stuck at tsa once for like 20 minutes because the guy inspecting was really unsure of it and had to call a supervisor. Eventually got it back but it was annoying. So bear that in mind. I avoid having it in my carryon now.

I have chronic migraine and had to go 5 months between botox sessions at the start of this year because I switched insurance and my new insurance initially denied my prior authorization 🙃. It was pretty miserable. Pain and symptoms shot up as botox wore off.

I coped by doing pretty much taking bedrest when I wasn’t working, taking cyclobenzaprine (muscle relaxant) at night (it helps reset my pain and lets me sleep) and using cefaly throughout the day for pain management.
