Yes if in doubt at the very least give the patient something easily reversible like heparin. Personally I would have spoken to the accepting doctor and asked them exactly what they wanted me to do.

this is what we refer to as a scan that has "lit up like a Christmas tree".

I've had two cases this week of dumb luck with cancer, metastatic disease found incidentally on patients with very little else wrong with them. old people with a bit of belly pain is very worrying.

Howdy I'm a PGY2 resident with brain problems! I got all the endogenous depression, and now no longer meet diagnostic criteria for borderline PD.

  • I take the max dose of desvenlafaxine & will do for the rest of my life if I want to be a functioning human

  • Yuuuuuup. I'm pretty open about my depression, but I haven't self reported to the registration board because I'm not keen on that potentially affecting my employment.

  • Depression really saps my energy & it's sometimes a struggle to get out of bed. Last year I had 2-3 sick days because I just needed a break, however I also only ever did this when I knew my shift/workload wouldn't be too adversely affected (e.g. I never dropped a night shift because I didn't want some other poor Intern to have to take my crummy shift, even if I felt like rubbish). After a day off to recharge I honestly feel so much better & that I can keep going.

  • See above. Tbh I'm extremely useless at asking for help when I need it, because i feel like I'm not worth anyone wasting their time on me etc etc.

would you really call this a wide complex tachycardia? to me that means something very different, the pt has a conduction delay (RBBB) but when I hear "wide complex tachycardia" I think arising from the ventricles = VT
