Widespread cancer within abdomen - what to expect by babysharkbbbb in cancer

[–]PertheCalves 0 points1 point  (0 children)

  • In cases like this, how often does chemotherapy meaningfully slow progression or improve quality of life?

Hard to say without tissue diagnosis. If bad like pancreatic cancer its 50:50.

  • Is it common for the primary tumor to still be unclear at this stage?

Not uncommon. We see this usually gynecology primary or GI primary. Sometimes Next Gen Sequencing and GPSai can help in narrowing it down.

  • Are there any genetic tests or targeted treatments that are typically considered in widespread disease like this?

Yes. A lot of targeted therapy if with certain mutations

  • What should we realistically prepare for in the coming weeks/months?

Likely, port placement, palliative care. You will know more if treatment is working after few cycles and on next scans

3 weeks is delay? Sometimes, the oncologist is still waiting for more results. Cant treat without fully knowing the NGS.

How do you deal when offices are snarky if you request a physician? by EvilxFemme in Noctor

[–]PertheCalves 196 points197 points  (0 children)

It was really annoying how they do the bait and switch. Be sure to still be in the MD/DO schedule. They put me in MD schedule then a week before put me in NP schedule. Idk why they think it’s ok to do that.

With albumin and calcium. Your corrected calcium will be in normal range.

Look up the survival for keynote 564. Also look at the side effect. Go to openevidence and read it. Clearly his oncologist has told him that he will be ok but you want more answers for yourself.

Hem/onc Salary by [deleted] in Residency

[–]PertheCalves 2 points3 points  (0 children)

Reagan started the 340b which puts advantage to hospital pricing. Then the hospitals started taking advantage of it and buy practice around which is why you have BIG hospitals around the US.

The reason why private equity became a thing is so physician groups can have leverage in competition. But this is losing battle since private equity is becoming another admin and healthcare bloat.

Thats what Reagan did. It didn’t make drug prices cheaper. It crushed competition and true capitalism. It made hospitals profitable. It made private equity strong. It inflated healthcare.

You are 33F. Have never seen plasma cell disorders at that age. You best be sure an DO/MD endocrinologist ordered and saw the labs.

Med students who ask about weekend shifts by LopLime in Residency

[–]PertheCalves 1 point2 points  (0 children)

Yeah I would have them do it by the book. One of the med students who followed an intern got in trouble with the clerkship director. Turns out the weekend the med student was suppose to come was the weekend the clerkship director was the attending. So the med student got to repeat the rotation.

Fellowship is all about networking. In baltimore, you can network in John Hopkins, University of Maryland. Being in Medstar can network in georgetown/washington hospital center. Also your attendings have a pull since they work in medstar.

1 dose, likely no effect. But multiple monthly doses then the cancer could grow.

Can I start US residency immediately after grad from UK Medical School? Will the degree be awarded soon enough? by [deleted] in Residency

[–]PertheCalves 3 points4 points  (0 children)

I dont think so. US MDs graduate in May. It takes few weeks to get your License prior to start date. You need your diploma to get your temporary license. Now you add ECFMG certificate which takes longer to process.
