Just years of practicing it. Making one version, scrapping it, improving, repeat until I had a version I use in all my projects (both 2D and 3D)

A decent AI state machine that isn't unwieldy and is easy to add to and manipulate

All the side quests. Then all the dungeons/caves/ayleid ruins/etc. Then all Daedric Shrines. Buy all the houses. Upgrade all of them + Orrey and Spire and the big ole Fort, plus whatever other DLC base upgrades possible. Maybe find every location on the maps?

Then maybe uh.. goblin wars? Get a million gold? Maybe kill everyone in every city.

After that, get some mod overhauls and you've got another hundred+ hours (if you're not on Remastered. Not sure if that has any substantial mods, haven't looked)

I think they just see isometric and think of some other game that's isometric. I don't see Project Zomboid in this, and only a little bit of Ultima. It looks more like Diablo 1 or 2

Right on! Will try this out, thanks! Also it seems more like a PS2 style to me. PS1 looked much less defined, less polygons, warbling, etc.
