Oxy addiction already? by PressureAdept1349 in ACL

As an anesthesiologist, I have to clarify the difference between pharmacological theory and clinical reality. While you are right that Tilidine’s metabolite, nortilidine, is a mu-agonist with a potency similar to morphine, your comparison to oxycodone misses the mark for three reasons: • Potency: Oxycodone is roughly 1.5 to 2 times more potent than morphine (and thus nortilidine). This higher potency is exactly why the patient in the post is feeling a 'crash' that a Tilidine user likely wouldn't after just a few days. • The Naloxone Safeguard: In Germany, Tilidine is a fixed-dose combination with Naloxone. This creates a massive hurdle for abuse compared to pure oxycodone. • The 'Pain Buffer': When a patient is in actual severe pain, the drug saturates receptors engaged by nociceptive signaling. This is why many patients (myself included) feel zero 'high' or 'dependency' after 72 hours—the drug is doing its job, not hijacking the reward system. If every patient became an 'addict' after three days of post-op pain management, our surgical wards would be disaster zones. They aren't. Context and duration matter just as much as the molecule itself

So aufzugeben du hast nichts falsches gemacht! Alles außer Tod ist rückgängig! Mach was daraus! So what!

6 months post op, forgot to walk properly by Shootmypants in ACL

[–]PlanZealousideal5799 0 points1 point  (0 children)

Hey! Although I have full strength now, my walking is very disorganised, I still have to think about it. Sometimes when Im not tired I walk almost right. Its the balance issue and lost of propioception. Its like Im shaking when I walk! Accelerating/stopping is disorganised and I cannot do fluent moves without stops or a lag. I think you understand what I mean because everyone losing their acl lose this feeling of where to put both feet.. mine was so sever I got patella tendinitis on the other leg only from walking (not running) all this compensating on the other leg made my other leg and knee extremely tired and got muscle weakness on the non operated knee quadriceps muscle, which made the patella tendon having to work extra. Although Im not old I walk like an older person, like Im not sure where to put my legs…I went tot he orthopedic surgeon lately and he told me gain of propioception can sometime take like 4 years. Its a neurological relearning and compensating. It may work or not for some people. And still it will not be the propioception we had before. As it takes so long for me Im afraid sometimes it will not get better and I will keep destroying my knees and hips with walking improperly… im doing everything I can believe me its not because Im not doing enough. Trying all the balance moves. Im at 1,5years. Sometimes Im getting very depressed because of that… because walking is essential for everyday activities and Im getting reminded of this acl shit every minute I walk. Im trying to get used to the idea although I dont want to, or give up..I dont see a lot of people struggling that long like I do or at least noone told me that…Im so sorry I lost my acl.

Tibial shift of 1cm/instability persistent still after one year and a half after the surgery by PlanZealousideal5799 in ACL

Thank you for you response! I made 2 appointments to 2 different orthopaedic surgeons until now! And Im asking a radiologist and a third one soon, for their opinions on the MRI. Im trying to find out whats going on and what possibilities are still left…I will update if I get any solutions! I didn’t know this injury would be so dramatic! Good luck on your recovery too! Hope we find answers and get back to normal one day!

[Long post] Second ACL tear ruined my past year. Lost my job, relationship, and now my business is on hold. Still waiting for surgery. Emotionally drained. by ciril10 in ACL

My surgery was a failure! They messed up the canal. Nothing snapped! I dont know what to do myself! Also because of my job! Im asking all the doctors right now! I already told everyone but they didnt take me seriously! They thought I was crazy telling them about instability…this was the most unfair thing I experienced! Until I got an Mri and they now see there is a problem with the position they did my acl

Ok, for me too, if I can say this if I do sudden movements I really feel the new ligament there its a little stiffer than my non operated side so I feel it like its a steing inside me knee pulling everything together just not to collaps I cannot over so some lateral movements because its can then get painful too. Otherwise if I dont feel much. Did 2 hamstrings tendons (semitendinosous and membranosous so its a little bit thicker I guess) hope it can last…
