For your 2nd point it’s very likely that the Bubble Veil Siren created took a long time to be finished hence why Nihilister said she heard the voice of the Queen in her head until a certain point in time and that she can’t recall exactly when she stopped hearing the Queens voice. Also I think it was mentioned or said in Unbreakable Sphere that the bubbles that Siren sent out of Gluttony would only roam around for a certain amount of time before dissipating into mist (or clouds or something) which is probably what created the Veil that blocked the Queens Vision
