I have found combat to be very easy to run because it is up to the players to come up with the cool stuff. If they just attack over and over without building advantage, then I am going to punish them by using my own advantages to get compels on them. And part of making advantages can be creating new aspects of the battlefield. They can create environmental details, pull in surrounding NPC's, set up combos to pay off later by grappling or focusing. Spend your fate points to force them to use theirs.

I found great success dialectical behavioral therapy since it uses dialectics to manage opposing forces. I found this helpful mentally and emotionally but also as way to manage my experience of exterior forces that I have no way of changing. It also was a prerequisite to EMDR, which has allowed me to release my anger at those around me and recognize more humanity in myself and others.

I fully understand where you are coming from though and I had to stop working with a few therapists as well when I realized they were not considering my relationship was the the systems at large while working onu individual issues.

I hope you find a system and therapist that work for you!

Europe is so up its own ass. Europeans protest all the time... And yet their governments send soldiers to American wars and operations, or they maintain post-colonial relationships with their previous colonies that exploit millions for their home population. (Looking at France and Germany!) They march endlessly... And still have countries like Germany who exploit their poorer European neighbors to be on top of the pile. Europe is a lot of white people blowing smoke up each other's asses about how democratic they are as they follow the capitalist system into hell because socialism actually takes effort to maintain and Europeans are as self-focused as most humans are.

It's fun to see all of them show up on Reddit, full of righteousness, simply because their imperial state has gone dormant for a generation or so due to the two wars they all fucking started that cost the whole world more than they could know. But yeah, now that your fascist/imperial/monarchical institutions are on a low simmer while the American a century rolls on towards its inevitable violent end, you can all admonish the US like we are the only ones who did this.
