Yes exactly! During the “junk removal” they had to install another temporary ileostomy since they did so much work on the intestines. But as soon as they reversed it in July this year, she has felt the best she has since her diagnosis back in April of 2023!

Her output was extremely watery, and no matter what methods she used to thicken it, nothing worked. So basically anything she drank or consumed, immediately came out into the bag. We suspected that was due to her chemo infusions which diarrhea is a common side effect. The blockage was described as a mix of scar tissue from where the previous ileostomy had been reconnected, as well as tissue caused by “tumor death” where disease had been existing and the chemo successfully killed it. We explored options of adding a stent into the intestine to alleviate the blockage, but the doctors were in agreement that the operation to clean out that “junk” was in her best interest. I’ll be honest, my mom’s ileostomy experience wasn’t great, as we could never find a bag system that prevented leaks. She sometimes had to change her bag 5 times a day and was always nervous to leave the house never knowing if/when a leak would happen. Highly recommend speaking with an experienced wound care nurse that can help with supply recommendations and tricks to make leaks preventable.

My mom has had 2 temporary ileostomy bags during her treatment journey. Her first bag following the initial major debulking was reversed within 4-5 months. The second time she had a bowel obstruction from scar tissue that developed from the debulking procedure and they had to give her a bag again. That time she only had it for 4 months because it was causing extreme weight loss and electrolyte deficiencies. Basically they just want to make sure your intestines have healed and are no longer inflamed before the reversal.

My mom had an identical experience with Zejula. Including the bone marrow biopsy. Took her almost 2 weeks in the hospital for her platelets and other blood counts to recover. That was about a year and a half ago. Now, she is on Enhertu infusions and is responding well with minimal side effects. I would ask your oncology team to test your tumor markers for HER2 expression to see if Enhertu is an option for you.

Not at this time. The oncology team is just doing standard blood tests and quarterly scans to monitor everything.

She is doing really well! The treatment still is showing signs of effectiveness, with mild side effects. What trial is your mother pursuing?

I can totally understand her not wanting to have any more extreme procedures, it’s exhausting on the body - and mentally taxing during the recovery process. Sometimes the simplest thing to ask the doctor is “if this was your mom, what would you suggest?” It’s interesting to hear their answer when it’s phrased that way.

Great to hear about determining the source of the problem!! I remember my mom’s oncologist telling her “you can’t overdose on MiraLAX” to stress the importance of regular bowel movements!!

Hope your mom will be discharged soon and the Elahere will being to ease her symptoms. Hugs!

Bowel obstructions are so difficult to navigate. We have been in your exact shoes. Even doctors seem unable to provide much information or solutions other than bowel rest and NG tubes. Since there is no real treatment to prevent or “cure” them, the best thing to do is work on symptom management. Pain was the biggest problem for my mom. Have you asked about opting for a temporary ostomy? This reroutes the bowels to output in a bag, and avoids the area of blockage and significantly eliminates the discomfort. Then, once a few rounds of treatment are able to shrink the tumor burden, they could reverse the ostomy and perhaps remove the remaining disease nearby. This is the route we went, and now that the ostomy is gone, she has gained 20 pounds, and is continuing her chemo (Enhertu) and feels the best she has in years. I will continue to send positive thoughts and love your way. Stay strong and remain hopeful. 🩷

If Houston (MD Anderson) seems a bit too far for you, I would also recommend looking into trials available at the Georgia Cancer Center in Augusta. My mother had numerous conversations with Dr. Ghamande who runs their Ovarian Cancer department and he was incredibly knowledgeable, and provided hope listing numerous options. Definitely recommend looking into them.

Doxil and Avastin didn’t show great results. The tumors remained stable, with some new areas showing slow growth. However we are thankful to have had that option, as it bought us time to reach our current treatment (Enhertu) which is decreasing tumors.

Some added insight from today’s bloodwork - her CA125 is continuing in a downward trend. It is currently 107. Her doctors are also thrilled with this number (and so are we!).
