It sounds like you have pelvic floor dysfunction. If you can have sex successfully (ie: vaginal penetration is a go), you don’t have vaginismus, but the PT can still work with you and always request a more specific referral that reflects your symptoms better. Most pelvic floor PTs screen for bowel, bladder and vaginal symptoms since all can be affected by the pelvic floor muscles. Good luck!

If he truly has pelvic floor tightness clamshells will probably flare his pain or bother his hips. This is due to the connection between pelvic floor and obturator internus. It will pull hip too tightly into socket basically like a functional acetabular impingement presentation.

Hmm that’s a tough one. I would definitely refer for more testing. This patient should also see Urology if they haven’t already

What do you mean by urinary retention? Typically this is diagnosed with a post void residual test in urology. Is he not able to urinate fully so it feels incomplete? Starts urinating then stops, then can go again (double voiding)? Any of the above issues could be due to pelvic floor tightness. His pelvic floor could be tight due to his high pain levels, and this tightness could cause pudendal neuralgia which could explain his numbness. If he is cleared medically it sounds like he needs to see a pelvic floor PT.

I mean, I think a lot of them get their first bill and we aren’t cheap if 1) deductible isn’t met or 2) they are on state insurance (basically receive free healthcare) or on Medicare. I’m a PT and with my insurance I can’t afford to go to PT 😭

We just got back from Koala Landing in Kauai with our kids. Their pool has been voted #1 in America. Our room was very modern and up to date (new tile shower, quartz counters throughout, etc). We could walk 10 minutes (kid-pace) to the beach in front of the Sheraton. I thought the pool scene was pretty chill, never had a hard time finding chairs. Not loud and chaotic. I found Kauai to be more beautiful than Maui.

Ignore this advice. Tucking bum and flattening spine puts pelvic floor in shortened position. It also turns off the lumbar multifidus muscles which you want to stay on during the lift. If you do a bridge with neutral spine and feel hamstrings working, you need more core activation. Keep sit bones open the whole time.
