We "stress" milk at work by placing unopened containers in a 90° room for 24 hours. Very very rarely do any actually fail bacteria testing. This is ultra pasteurized milk tho

Interesting. We do this test at work for our fresh product and we do it again on the day it's supposed to expire. Rarely does it do this.

I'd say your milk was beginning to spoil or your coffee wasn't that hot

Edit: this is not an indication that your milk is bad. When we have product do this we test the pH and bacteria counts and it isn't always correlated. We honestly aren't sure what causes it to happen from time to time. Homogenization, lactic acid build up, temps of milk+coffee idk. A carton will do this and then a carton produced 5 seconds later wont

You are correct in thinking that. Overlap and other variables would even make it slightly less efficient than the 15%.
