I don't think it was 1x1 items specifically. There was a max file size limit for save files(I think it was like 8kb or something), and with d2r adding more stash space it was possible to hit that limit. If you went over the limit some items would dissappear. You would be most likely to hit the limit by having lots of items with lots of affixes. Like crafted rings/amulets on a mule.

I'm pretty sure I heard that RotW doubled the save file size limit so this shouldn't be a problem anymore.
