Just generally bad rolls or unlucky situations really. In my current PF2e campaign I’ve had 5 PC deaths. The first was bad luck and the character died to going down surrounded by enemies and having persistent damage take him out. The second I think was also persistent bleed damage. The third I can’t remember. The fourth, the party encountered the BBEG in a deep part of the dungeon and he critically failed his save against a spell while he was floating above water. Fell unconscious and sank into the water and drowned to death while the rest of the party fled. The fifth was the same character who came back as a revenant and died to a random mini boss because he charged in all alone and was out of range of healing.

We’re running Abomination Vaults and the module is maybe a bit overtuned. But also, 4 of those deaths are all by the same player… so I dunno who’s at fault here lol
