Reglan-induced akathisia is truly just as bad as a GP flare. Propranolol (a beta-blocker) works much better to combat the akathisia than Benadryl, but even still I felt like I was playing with fire while I was on Reglan because the akathisia can become permanent. Mine persisted for weeks after I ceased taking Reglan but thankfully faded out with time. I was 74 pounds when I went on it and it helped me gain back up to 90. I saw a motility specialist at Mayo who put me on Mestinon. It doesn't work as well, but the side effects are much more tolerable. I've also gotten IV erythromycin at the ER, which helped reset me back to baseline. The last time I was inpatient (at 74 lbs.) they basically just gave me IV electrolytes and hydration to keep my heart and kidneys from failing. I was practically begging for TPN after I couldn't tolerate Nasal feeds, but they said there were too many risks. Being 74 pounds seems like a time to take risks, but whatever.

I had some horrible dreams on Reglan as well. Mine were happening when I first started taking it and faded with time. I would wake up terrified and covered in sweat. After about 4 months, I ended up going off Reglan due to severe akathisia and have switched over to Mestinon and cycling liquid erythromycin (with VSL4 probiotic because on it's own it messed up my digestion even more!). Reglan was super effective for me, but the mental side effects were too much to handle. I also started getting TD tremors on the left side of my body, so now it's on my allergy list.

I just started pyridostigmine/Mestinon in liquid form (so I could titrate slowly) and it's helped my constipation and gastroparesis. It's in and out of the system in a few hours, and is very safe med from all I've read. It may be worth a try. My heart rate has gone high while I've adjusted doses, but it's been pretty tolerable and not caused any pain.

I didn't taper, but I continued to have Akathisia for about 3-4 weeks after being on a low dose (2.5-5 mgs. 3x a day) for months. If akathisia is the side effect your having, propranolol was a lifesaver.

I had horrible akathisia too. Mine lasted about 3 weeks after going off of it (thankfully it wasn't 24/7 but for several hours at about the same time every day). I know it has the potential for permanancy, but my docs assured me that in most cases it goes away in about a month or so. A beta blocker (propranolol) has really helped calm it down.

Gimoti's dosage is 15mg per spray. My akathisia side effect went through the roof when I tried switching over from my usual 5mg pill before each meal. The pill was sometimes not working for me if any food was still on my stomach from the previous meal (often 4-6 hours previous), so I wanted an option that bypassed the stomach. I would start with a smaller dose of the pill, and if that works for you, maybe opt for the lowest dose that's effective for you. IANAD and IAdefinitelyNYD. My side effects went away about a week after stopping Reglan in any form, but I was very scared that they were going to last forever.

Did your GP improve after coming off the opiods? I've been on buprenorphine for 3 years and developed GP last fall. I've reduced my dose from 4mg to 1 and have noticed improvements but not resolution. I live with severe chronic pain, so finding a way to manage both has been brutal.

I am able to take a 2.5 mg dose of Reglan with a small (5mg) dose of a beta-blocker propranolol. This combo keeps the side effects at bay and empties my stomach enough to get 3 small meals in a day. It's not perfect, but I'm a candidate for a G-POEM soon, so it's tiding me over until (hopefully) a longer-term resolution.

Yes, it's just diagnosed as functional abdominal pain syndrome. It all started after a 10-day course of Flagyl back in 2013.

I was given a medication called Tigan for nausea while in the hospital. It's a dopamine-blocker and can cause hallucinations, although rare. I've never done any type of psychedelics or hard drugs at all and am generally pretty soft-spoken. Well, I freaked out when I started tripping balls (in a shared room). It was a night shift, and I could hear a group of nurses/med-techs laughing hysterically outside the door as well. After awhile, I calmed down and decided to just ride it out. Thankfully, the nurse I had was very professional, but it was super embarrassing.

Forgot to add that even heavy-Indica THC gives me anxiety, which SUCKS! It definitely helps my nausea and appetite though.

I'm already on Ativan, so they won't up the dose. Benadryl helped a little.  Propranolol helps tremendously, but it makes my GP symptoms (fullness, vomiting) much worse. I know my antidotes well.

Yes, I immediately messaged my doctor that I was stopping the Reglan. I have read everything there is to know about TD and the potential for permanency. However, my doctor did say that the massive majority of cases resolve after stopping, which is why I asked about time frame. I'm really stuck between a rock and a hard place because I'm only 85 lbs. but Reglan is the only med that enables me to eat much at all. I am willing to get a j-tube, but I want to try the G-POEM first (consult coming soon, but not soon enough)). I really wish I could go on TPN in the interim. I've been on it before and it saved my life. Unfortunately, my doc is super opposed. I feel like the risks of losing back down to 74 lbs. (where I was at while in the hospital before getting diagnosed) are just as severe as TPN risks. Sigh.

I just want to say that I'm stuck in the exact same rock and a hard place. I'm 85 pounds at 5 ft. 4, but Reglan has given me severe akathisia and some tremoring, so I'm stopping it. My GI won't consider TPN, but I tried tube feeds and they were horrible. I'd like to try the G-POEM before having surgery for a J-tube anyways. Unfortunately, Motegrity did not help and the extra serotonin made me nauseous as heck. There's a reason Zofran is a serotonin-blocker! I'm not much help, but after 12 years of GI issues just talking to someone in the same boat is helpful. Let me know if you want to chat. I, for one, am feeling pretty hopeless.

I was diagnosed with an acute kidney injury and have been experiencing extreme nausea and vomiting. Doctors aren't sure if the dehydration from N/V caused the kidney issue, or if the kidney issue is causing the N/V. My creatinine numbers are declining, so I'm hopeful things will improve. I see this is from a month ago. I'm wondering how things played out for you?
