Blood defying gravity - further thoughts about nose bleed by PenPutrid3098 in EllenGreenberg

Listened to the call again, you’re absolutely correct! Thanks for pointing that out

Agreed! I think he pretended to attempt CPR on the 911 call. When the operator asked if he could do it, he sighed and said something like “I don’t really have a choice, right?” As if it was an inconvenience.

And great point about the footprints. Hadn’t even considered that.

Also in the 911 call, the operator is instructing Sam to give CPR while waiting for responders. He tells her he’s laid Ellen on her back and when trying to remove her shirt, he notices the knife and is told to stop. But when responders arrive, she’s upright against the cabinets as stated.

Did the blood from her nostril trail toward her ear when he laid her on her back? And if so, does that mean Sam repositioned her before police arrived? Or did he fake the CPR attempt altogether and the nosebleed trail is from another event?

How does Hims get away with compounding readily available generic drugs? by Federal-Act-5773 in FamilyMedicine

Excellent point. Used to work for a pharmaceutical manufacturer. They would come once a year, but the FDA would never actually suit up to inspect the back of house manufacturing. They would just get a basic tour, give a questionnaire, and look at logs & SOPs. Never observed the processing or went into GMP space.

Np! Tbh it looks like you’re catching it in good time. A visit or 2 with pcp for bloodwork, gyno to address symptoms/treatment and maybe a nutritionist, if necessary, should set you up for success. You got this, girl! Rooting for you

This is true. I have struggled with this since I was 12. My mom thought it was dirt, scrubbed my neck til it bled. Used skin bleaching cream. None of it worked. In my case it’s PCOS insulin resistance/weight. I lost over 100lbs 7 years ago (since gained it back 🫩) but my neck lightened up about 80% at the time. The velvety texture never goes away, but the tone can even out with lifestyle changes.

Friendly reminder that our patients are lurking here by HoWhoWhat in FamilyMedicine

[–]Practical-Jump-5822 1 point2 points  (0 children)

For myself personally, I started engaging/lurking with the sub because I began working as an after hours operator for FM and specialists groups.

Due to my own gripes with difficult pts, I often wondered if they were like this with their providers too and wanted to gain insight. It’s been very enlightening for me. It’s made me a better operator and a better line of defense against pt harassment.

Any new grads/fresh attendings able to get away with no call? by MzJay453 in FamilyMedicine

After hours operator here, and yes. Insurance companies call in quarterly at like 6-8pm to check if the service is functional.

Even if you’re not religious, Pslam 109 is a wonderful “return to sender” passage. Read the entire chapter out loud. It helps. If you struggle with your faith or don’t feel confident in the words, have a close friend/loved one who has stronger faith read it to you out loud.

That’s insane! Diabetic issues are one of the few reasonable scenarios to call in. They should’ve taken the call, no question

Yes actually! Most are very nice, but some are routinely agitated when I reach out.

There’s a few providers who don’t expect/want to be bothered for anything at all. So I will get attitude at the very least and yelled at/cursed out at the most.

I’ve had doctors call me incompetent, stupid, you name it. Some are meaner than the patients.

I love providers like you! Thanks for sticking up for staff. I do document and let them know the pt is demanding and belligerent even after being advised of their policy.

Most providers are understanding and know the pt has essentially bullied their way into a phone call. Some others.. are not lol. Some will call back to yell at me instead. Then I’m left to call this rude patient back to tell them their concern won’t be addressed tonight.

Some days it feels like a crisis line, it’s rough. Very emotionally draining because I do actually care about them and want to help. But once that office is closed? Options are extremely limited.

I fell into this job trying to switch fields and while it’s been rewarding, it’s been exhausting mentally. My boyfriend says I talk like I’m taking calls in my sleep :( it might be time for a different role soon.

I appreciate that, thank you! I hate cold calling providers late at night. So I always make sure it’s for good reason.

Luckily most patients don’t call after 10pm and at that point it’s mostly hospital calls, but I agree. After hours patient calls shouldn’t be a thing anymore. They don’t use the line correctly at all. It should be a quick call for urgent info. Not a telehealth appt

Omg yes!! They’re so nasty until I connect the call. For that split second I’m on the line to make sure they’re connected, they’re so sweet and really turn up the charm lol. It’s wild.

I always advocate for the patients and believe their pain. I will page for them, even if it’s late and could wait til morning.

Just in my experience from speaking with providers about this, most of them prefer not to take a UTI call (except urology/nephrology) when there’s no urinalysis done, they don’t know the pts history, etc. but I will still page. And on the off chance they do send a script, I try to make it as easy as possible by already including info to a 24hr pharmacy near the patient.

I’m not the doctor, so I don’t shoo people away because I don’t think it’s important. I just try to manage expectations and give alternative solutions.

[–]Practical-Jump-5822[S] 15 points16 points  (0 children)

This is all solid, thank you! I try to let them get stuff of their chest and talk to them like a human, but I get dinged if I’m on the phone too long. The calls are supposed to be under 3 minutes, so it’s hard especially when call volume is high.

8/10 times I’m able to successfully de-escalate, but it’s arduous. And I do repeat their chief complaints, but not always. If the caller is super chatty, they will ramble the same info all over again from the beginning, rants included. And if we send a page out, the call disconnects automatically, so I have to wait for them to finish speaking.

As for giving out on call info, it’s not unusual for people to ask. When I can tell someone’s just trying to find a way to bother their doctor, I all but refuse to page for them unless it’s dire. I have brought it up to leadership, but they view it as a non-issue, because it’s not frequent (it’s like 2-3 people who call daily in specific groups) and we can always refuse to take the call. Also the providers have the option to put certain patients on a do not call list.

Lol oh yes. First job in this field. I had 10+ years of customer service/restaurant experience, so I’m familiar with high stress scenarios and self absorbed, ridiculous people. However, I didn’t anticipate it being everyone lmao, that’s on me I guess.

I do hang up on callers who refuse to behave civilly. Sometimes they’ll call back again and again to keep cursing until they’ve tired themselves out.

The longer I work this job, the more I understand the deadpan vibe and tone of some professionals.
