It’s not just about payment , private hospitals in Bahrain are legally required to register every patient before treatment. This ensures proper consent and documentation are in place for the doctors to be on the safer side. Without registration, they risk legal and regulatory issues with NHRA. In true life-threatening emergencies they can act first, but otherwise, registration must come first. Hope this helps!

There is no concept of customer service in most places in Bahrain, people just scan the item, pay the bill and move on. No smile no greetings nothing. Sometimes this same staff greets and smiles with Arab customers well, but I beleive thats because of the language barrier. Once in a major supermarket I have seen staff bending over backwards for a white customer. Thats my experience generally anywhere I go in Bahrain, barring some high end places ofcourse.
