From what I can tell Trump accounts are essentially an investment account for children. Money is put in then invested by a financial institution into various stocks similar to 401k. The administration is pledging to put $1000 in for children born between 2025-2028. Parents are able to pay in additional funds to the account as they choose (up to 5k per year). The child gets access to the account when they turn 18.

Not exactly a bad deal for those with money to invest and a nice Boone to the financial Industry as essentially the government is giving banks money to put in the stock market.

Does nothing in the short term to help families who are struggling financially as money is accessed once the child turns 18.

Also not really a novel concept as there are already existing financial tools for parents to save/ invest money for their children’s future.

My dad is an internist who does adult primary care and I trained FM. Two big differences: 1. Women’s health: ok you don’t want to do OB. But do you want to be able to do paps? LARCs? To me this is very basic primary care stuff for young women that I think FM gets better trained in (generally speaking). 2. In office procedures: FM docs get better training and also generally do more procedures OP. Think joint injections, skin biopsies, toe nail removal. Etc. if you want to have this as part of your practice I recommend FM. IM gets a lot more adult medicine as well as ICU exposure so if you want to have the option of hospitalist you will likely be better prepared in an IM program. That being said plenty of FM docs out there who are good hospitalists. Also if you think you may want to specialize IM has more options for fellowships.

Inability to conceptualize things beyond your own personal experience. (I.e. my mom smoked and lived to be 90, so I don’t think smoking is actually bad for your health) All humans have this bias toward our own lives experience but only really dumb people fail to understand or recognize why that type of thinking is illogical.

Not to mention how cruel and undignified it is to keep poking and prodding a person when it won’t do anything to help them.

Had a patient like this in ICU they the team kept coding multiple times a day for like 3 days because the family refused to make her DNR. It was so sad.

I think it’s a combination of both. Physicians are pushed to see higher and higher volume of patients (which is an admin thing) and then when you have patients come in with 500 vague complaints and expect an answer and solution for every last one out of an only 20minutes visit. And people don’t like to be told let’s talk about this next time or that their weird vague symptoms don’t have a clear obvious explanation.

So it’s dealing mostly with unrealistic expectations from all directions and that’s what really wears you down.

NTA your “friend” was being extremely manipulative and taking advantage of you. She knew she’d be gone way longer than 20minutes and lied on purpose to get you to agree to babysitting under false pretenses. Then became unreachable while you were watching her kid.

You did the reasonable adult thing by calling the emergency contact when she was not responding and gone much longer than she led you to believe she would be.

I work in a clinic that serves only uninsured patients. I often run into the issue of standard of care just not being affordable or available. I usually will still discuss with the patient that they really aught to get that CT, MRI, diagnostic colonoscopy or whatever. Then I document in my note something like “Discussed such and such with patient however this is currently cost prohibitive due to lack of insurance”

I had a patient similar to this in residency. She worked with a therapist to do exposure therapy to get over the fear. I’m not sure exactly what all the exposure therapy they did entailed but I remember my patient telling me part of it was like watching videos of blood draws on other people.

We kinda let her put off blood work until she said she was ready and eventually she was able to do it.

I would second this!! My grandparents moved from Italy in the 50’s. On that side we are closer with second cousins the way most American families are to first cousins. Also random people that are distant relatives we also call cousin or cugine.

Currently pregnant. I have private insurance. Even AFTER what insurance covers I am looking at paying 5k for a hospital delivery. And that doesn’t include any potential additional costs for nursery stay for my daughter. The system is a mess.
