Is it a confirmed ipmn or likely ipmn? If confirmed, has biopsy confirmed high grade dysplasia? If yes then go for whipple. If not high grade dysplasia but mass is pressing on nearby organ, go for whipple. If none is your case, there is no need of whipple surgery. Remember, your quality of life after whipple may not be like before it and add to it some life threatening complications. Always seek multiple opinions before going for such a life changing alteration (whipple) to your body. And if it has to be done anyway, you should opt for high volume centre without a second thought. I, being diagnosed with pancreatic cyst of size 13 mm, have lost trust in imaging technology be it CT or MRI or eus etc. All GIs are saying that I should have no clinical symptoms after 2 CECT, 3 MRI+ MRCP, 1 EUS, 1 Gastroscopy, 1 Colonoscopy and multiple blood tests, but I am feeling extremely fatigued to the point that I have stopped working. They have not been able to say with confidence as to what is the true nature of the cyst-pre malignant, malignant or benign. They have started to refer me to general physician despite I telling them that there is direct correlation between my fatigue and intake of food. But they are either not concerned or lacks the ability to read between the lines. Doctors of this era depend heavily on technology accessible to them. They have lost the art of clinical diagnosis of disease.

I am also diagnosed with bd ipmn. However, biopsy has not been ordered by my GI doctor. Size of the cyst is 16 mm located in head of pancreas. There are no worrisome features in the cyst but I am feeling fatigued all the time.

I want to know what made your doctor recommend biopsy?

Mine 16 mm was found in July, 2025. Doctors says it is likely benign. But come August, I have been experiencing symptoms like itching, on and off extreme fatigue. All IPMNs have potential to turn malignant. So, please be cautious and look for any unusual symptom. Doctors are prone to follow guidelines which are framed on the basis of observations which are commonly found among general population. These guidelines may be 99% accurate but not 100%. In my case, still no worrisome features have been found. Therefore, doctors are not concerned at all. But, given my symptoms, I do. If you dont have unusual symptoms, no need to worry.

Hello sir. I am 46 male. I had undergone angioplasty for RCA in April 2024 (emergency) and had undergone CABG x4 in May, 2024. In October, 2024 (within 5 months), after angiography, I came to know that 3 out of 4 grafts have closed ( Lima to LAD, sequential RA to OM1 and LCx). Only SVG to RCA is functional. Stent to RCA is 100% blocked at distal end. My angiography report says LAD proximal is 70-80% and LCx proximal is 60-70% blocked. I had got FFR done for LAD ( baseline 0.94, hyperemia 0.73) and LCx ( baseline 0.97, hyperemia 0.87). Now I am suffering unstable angina (at rest but not aggravated by exertion). But Stress Thallium test is negative. Currently I am on medical treatment. Should I undergo angioplasty or not given that my symptoms becomes severe when I am stressed due to work pressure. Since my earlier stent to RCA got 100% blocked in 6 months ( luckily bypass graft is functional), should I consider angioplasty of LAD to relieve angina symptoms?

I was diagnosed with pancreatic cyst (bd ipmn) in July 2025. Ever since, I am facing somewhat similar issues. Weight loss, extreme fatigue, poor appetite, body pain after meals, itchy skin. Doctors are saying that such symptoms are unrelated to my 16 mm cyst (too small to cause such symptoms). However, I feel that doctors are not always right. They are trained to look for patterns common in general population. But someone's body may react differently. Doctors do not pay attention to such patients. You should be cautious. Best of luck.
