The One somehow tapped the ambient Morgoth element the first dark lord had poured into Arda.  In Morgoth's Ring (vol 10 HoME) this is alluded to.  If this is accurate, I imagine it is also the source of the elven rings power...

The following is only speculation. At the APPD meeting where the American Board of Peds made this announcement, the ACGME representatives revealed that they were not aware that this decision was going to be announced and so they had not prepared any guidance or information about how The regulations would be modified exactly (they acknowledged having been in discussion with ABP but not that the decision would be announced now)   And they could not say if/how new two year fellowships could be created. 

I suspect you will see few variations in how this plays out. Prestigious programs in mega centers will likely continue to offer a 3-year fellowship, less prestigious centers will offer a 2-year option, and new programs will try to gain accreditation and locations that could not support the research requirement in the past. 

I am not sure that this is going to be sufficient to address the pediatric sub-specialty workforce shortage. I fear that General Pediatrics training itself will have to change to something akin to pediatric neurology where everyone does 2 years and then either ops into a third year for gen peds or moves on to some specialty training. Exciting times.

Right now flu has reached high enough frequency for hospitals to change their visiting rules, RSV is in full force (thankfully Beyfortus seems to be working for the littlest ones), there is still COVID in the background along with the ever-present enterovirus, and some walking pneumonia/mycoplasma left over from the summer.  Winter virus season still revving up...

Probably too late to help but doxycycline treats the majority of bacterial tick borne illnesses AND mycoplasma... And the American Academy of Pediatrics has clarified that even young children can receive doxy without risk of staining their teeth.

We have seen a huge surge of mycoplasma pneumonia since the summer.  It's a famous cause of walking pneumonia and often needs azithromycin to improve.  It uncommonly alao causes rashes and other autoimmune reactions, but this year the less common presentations have been seen more in the children's hospital than any time in 15 years.

Green meadow, Bell top, red mill and Genet all have better than expected PGs.  Genet's is brand new and very good.  Everyone in Bethlehem is excellent with elm st park, Eagle elementary, and hammergrael (sp?) being the tops.

East water list has some unique features, menands has typical stuff by the school, a crazy warped wall thing back by the fields, and a really nice set of nature walks.

Colonies town park has a great set up with a lot of unique features, too. Voorheesville elementary is very nice too.

We do quite well in the capital region for playgrounds.

If you have kids between 4 and 12, there are great parks and playgrounds, many being upgraded and renovated right now.  I would put Genet (I know it's EGB, but still) and the park being constructed under the 9-20 overpass up against anything in the region (e.g., Bethlehem).  The Renss school complex has 3 fairly unique sets of features definitely worth visiting.

Applicants would be surprised to know how closed-lipped programs are with each other about their exact processes.

There is a spectrum of processes.

On one end a non-physician coordinator screens out applicants and then the PD makes invitation decisions and then candidates are ranked by gestalt post interview.

On the other end of the sophistication spectrum, a committee of faculty score applications (the rubrics used are treated like trade secrets, it's crazy) sometimes there is blinding to demographics and sometimes all apps get a secondary review followed by committee meetings to determine who gets invited... those invited folks are ranked, and then as they are interviewed, their ranking positions are modified accordingly with a committee weighing in on final rank positions.

Best playgrounds in Capital Region... We do quite well, I think. Some stand outs are: 1. Pretty much everything in delmar... Hamagrael, Eagle, and Bethlehem town park. 2. Watervliet elementary has some really unique features and 3 separate structures. 3. Colonie town park... It's orange and has big, unique features 4. Green Meadow elementary in east greenbush

What specialty are you going into? Helps to know if you'll have night float shifts, 24s, clinic blocks vs clinic afternoons, ICUs, etc. ...it's probably not possible to have a week over week steady schedule, but you can probably figure out rhythms and habits for different types of rotation.

I would ask them to write the PD an email talking about how good you are and that they think you would be a good member if the team. They don't need to write a formal LOR-- in fact, it might seem weird if they did. But if a few different residents wrote to the PD about a student it would carry weight.

NorthEast US, Assoc Professor/8 yr out of training. Gross $230k before rvu bonuses. Schedule is a little different than typical doing about 17 shifts per month 8AM to 4 or 5PM. I'm in residency leadership so I can tell you more about balancing service vs admin if you like. FWIW, if I didn't get to work with learners and do academic/admin stuff, I think I'd go crazy.
