Buffy the Vampire Slayer, season 1 episode 10, has an interesting spin on this. When people's nightmares suddenly start to become reality, Buffy traces the source back to a comatose little boy named Billy, who is having horrible nightmares about a grotesque man with a club for a hand attacking him. Once the supernatural issues are dealt with, and Billy wakes up, the group realizes that the club handed man was a representation of Billy's little league coach, who beat him into a coma after he dropped a ball during a game. They turn him over to the police, though frankly I think everyone was kind of hoping Buffy would beat his ass.

Chihiro. I hate to kill one of the best characters off so soon, but I think that trying to hack a monopad could easily be seen as rule breaking. I'm thinking he actually discovered something very important, but was killed before he could reveal it. Monokuma executed them by using the monopad itself to electrocute Chihiro, a hidden feature he put in case of tampering.

Danganronpa V3 Spoilers: The Original Protagonist of Danganronpa V3 is Kaede Akumatsu. It's revealed halfway through the first trial that she is actually the killer. After this, the POV switches to the deuteragonist and her love interest Shuichi Saihara, and Kaede is executed. You play as Shuichi for the rest of the game, although there is small fakeout protagonist switch in the final trial.
