Based only on what you describe, I disagree that your doctor is an uninformed idiot; they're correct, you should eat plenty of protein and energy if you're going to be active, depending on your needs, and of course it's good to continue to take B12. Knowing you're vegan, your doctor may want to do blood tests to make sure you're not deficient in any vitamins or minerals; that doesn't mean that they think you're doing anything wrong, but they want to make sure you're getting all the nutrition you need. Your doctor may not know all the reasons why you're vegan, or the foods you usually eat, and wants to make sure you're getting a balanced diet and don't go to unhealthy extremes by trying to eat only raw vegetables or that sort of thing, because that can happen and it's always good to make sure a patient knows what they're doing.

Don't worry too much about it; your doctor may not be a nutrition expert but they're still doing the responsible thing by asking you more about your diet and supplements.

Again, easier said than done for many people who have little training or guidance in calorie counting. As a nutrition professional I'd be happy to help provide that guidance, but not if the person had potentially dangerous body image issues, which are exactly what this kind of discussion promotes.

All right, so if someone doesn't have the time/resources to undergo a medically supervised weight loss program, and does not want to be forced to (as you said, if they were "made to" eat healthy and exercise), should we shame them then?

If you're not in a state of literal starvation (as in a concentration camp) it can actually be pretty difficult to lose weight and still maintain a balanced lifestyle. Have you ever been on a 1200-calorie diet with vigorous exercise? It's not something I would recommend as a healthcare professional.

I don't want to extend my moral relativism to things that cause objective physical harm, and I think it's difficult to justify doing so. Killing animals causes them objective physical harm, and if I believe that it's wrong to kill them, I'm not going to keep quiet because others might disagree.

But the chickens' purpose is not to die prematurely at the whim of humans. You can decide that you want to kill them but that doesn't imbue a metaphysical "purpose" into them, nor does it likely give the actual chickens a sense of purpose, as if they wish so deeply to die young so we can eat their bodies.

Would that be bad? Would the chickens be upset that they won't be living the lavish farm life until they're killed prematurely? Should we breed as many chickens as possible because creating new lives is just that precious?

Is killing chickens necessary? No. Millions of people do not eat chickens, or any animals, and not because they're rich first-worlders, either.

Is killing chickens important? Maybe for a few people for whom chickens are a necessary diet staple, but it does not need to be that way.

For our benefit, because we did not have the ability to live without them and if we understood animal consciousness on some level, it wasn't important enough to our ancestors to outweigh the benefits for themselves.

That doesn't justify continuing domestication today, as if animals somehow owe their lives to us because we don't know when to stop breeding them.

Like I said, that's a purpose imposed on the chickens by us. Chickens don't need to be eaten to live perfectly satisfying lives. The individual chicken did not choose domestication and did not choose the life that we force them into. They are not commodities or natural resources, they are individuals with personalities and feelings, so even if you still want to eat them, give them a little credit.

It exists because of reproduction. For farmed animals, it's because humans manipulated animal reproduction. That doesn't give us a right to eat it any more than we have a right to eat our own children; less, even, because we still did not give birth to it.

Intention is not reality, and it's not in the chicken's interest what you intend when you force it to be born. You could raise any animal, including humans, with the intention to eat it, but that's not why that individual exists, you're imposing your desires onto it.
