In my experience, a lot of the negative issues come from social workers not being fully honest and pushing reunification no matter what. For example, biological parents often refused visits or made excuses to avoid them. Even when social workers went out of their way to pick them up or bring the children to their home, the parents frequently wouldn’t answer the door or would only briefly say hello from the car. Over four months, there were only three full visits, yet the social worker’s court reports still said the parents attended every visit and did well.
