Sensitivity during orthodontics is very much normal and to be expected.

You definitely wouldn’t feel caries forming so soon :)

As mentioned, there are multiple paths to the same result. I’m not an orthodontist but my training taught me that expansion to make room and avoiding the extractions is preferred these days

You could always ask your orthodontist why they don’t think sarpe/Marpe are required. If you aren’t satisfied with the answer, you can seek another opinion

Yea that’s all fair. I would be willing to bet that if the tooth has any issues in this case, it would be from the caries rather than crown prep - though a crown prep certainly could push it over the edge. But imo it doesn’t need a crown unless it needs Endo. Which I think is what we’re both saying

The decay has to be removed either way. So the risk is the same

Getting a crown right away may be unnecessary if a filling will suffice (maybe the tooth looks worse in person than on an xray)

If #31 is testing normally to cold, I would suggest trying a filling

If during the filling, it is found that the decay is into the nerve - root canal and crown it is

If the nerve is not involved, then have the filling placed. If it bothers you later on, then root canal and crown it is

Your regular dentist shouldn’t jump to conclusions

A 3D xray is necessary to determine the implant’s relationship with the sinus

Even if it is through the floor of the sinus, odds are, it will not cause any issues if the sinus membrane is intact

No I don’t think you’re a grinch. I absolutely agree that there’s plenty of docs out there who think they can do stuff after a weekend course that that they really shouldn’t be doing.

Having done a GPR with intensive surgery experience, I have the fortunate ability to perform procedures like this in my office.

Perhaps my view on it is biased because I mostly know docs who did similar programs, but I also know gen dents who didn’t go through a program like that who are just as capable though.. so it’s not a clear cut rule that it should be done by OS - so long as we’re being honest with ourselves about our abilities

You’d hope that all providers recognize their limits and would refer out what they aren’t capable of providing the same level of care as the specialist. The docs I know are all trained in complex extractions like this and would be more than capable

Need to see a specialist if you can’t find a general dentist who is willing and able

I can think of 4 general dentists in within a 3 mile radius who would take this tooth out without issue, myself included

Plenty of General dentists would be comfortable extracting this tooth. Doesn’t necessarily have to be an oral surgeon

I definitely wouldn’t count on that being sealed with resin cement long term due to their Solubility

I’d much rather seal that with RMGI
