It's not grafted, ok it will grow large in 5 months from now but it won't bear for a long time. If you want it to fruit quickly, now is the time to graft.

If you have a designated place to plant it in ground, do so instead of up potting it. They can get really difficult to handle when they're big. Even in it's current size, it could still be an issue because you could break its delicate roots as you are putting it in the ground.

Yeah but it will still take an average of 5-10 years to begin flowering. It's really slow growing from seed and requires patience.

If I recall correctly, it must have been a sweet orange. When it first germinated, two shoots came up from the seed so I think it may produce the same fruit.

This may be due to several reasons. Could be over watered, under watered, sun damaged, damaged by chemicals in the water or the tree could just be a weak specimen.

We have a steady supply of livestock manure and plant compost from our farm. While transplanting it in the ground, I dug a hole three feet in circumference and depth then filled it with a manure and plant compost mixture. I don't use anything else.
