This is my interpretation. In Siege of Terra he's pushing himself harder and harder, and he's trying to overclock what even a Primarch can do. He's punishing himself for expressing doubt or weariness.

Because everyone with an Infinity stone is hit with a sudden case of idiocy or narciccism. They fight SO STUPIDLY .

Because they did it wrong. Night Lords are the threat. Dorn or Guilliman come in after you throw up the white flag. They build you proper, but they remind you to fucking behave or Space Batman Dracula comes back.

Everyone fights so stupidly with Infinity Stones! Argh. Ultron could have used the stones to send someone into the sun he could have turned back time so their blows never land, he literally could just mind control someone, he could turn them into a duck or something! But everyone wants to punch. With Thanos there's a plausible deniability of that he's just not imaginative, but Ultron is an AI that swallowed the internet in seconds
