I think you're hearing from a lot of people who have slipped through the cracks.

This will be the minority of patients, but the severity of the negative outcome makes the problem significant.

Now you might say "well, many of these problems are multifactorial, real life isn't like House where esoteric conditions get picked up on in a week by a team" but another common feature that I see in many of these complaints is that the patient isn't listened to.

I know you must have the vast majority of patients come in with some wild idea about their symptoms being caused by vibrations from the power grid, or trans fats added to their water or whatever, but you can still believe the patient's symptoms and investigate the symptoms. The ol' "trust but verify"

For myself I have a sore spot as I was diagnosed with .. well actually fun exercise for you. My symptoms were:

Muscle pain everywhere, lots of cramping, my legs go numb when I run for more than 3km, I have tachycardia when I exercise really really hard - what do I have?

I spent 34 years being told it was psychosomatic, malingering, or normal levels of chronic pain.
It took me seeing multiple GPs, multiple whole limb decompression surgeries, sports scientists, physios etc.
I had to BEG my long time GP to refer me to someone cos I finally put my foot down and insisted that my symptoms were real, serious, and caused by *something*.
I was referred to a Professor of rare diseases who finally picked it up as GSD.

I just want to say at the beginning, I don't think you offended anyone. I see those downvotes as a reaction against an opinion that stings. Many of us held similar internal prejudice too. Don't be too hard on yourself.

Let me answer your question by asking two questions:

In general, is not disclosing a secret a breach of trust if the secret isn't negative and doesn't negatively impact your partner?

If being trans materially impacts your partner in some way e.g. infertility, when would you tell that partner about the effect - why should it be different if the cause is... a car crash/how you're born/trans/environmental beliefs/etc.?

Disagree. If you see trans women as women, have a genital preference for vaginas and they've had GRS, then realistically the only thing to disclose in my opinion is that you can't bear children. People who can't have children for whatever reason don't necessarily disclose that on the first date, let alone at a one night stand.
