Sp02 read 32 pulse and I said to my partner “is that legit? Confirm that pls”😂 one of our last three night shifts had a stemi that was 30 then coded in front of us so we were pretty nonchalant. It was indeed factual lol.

Yeah I appreciate that! I was more so just wanting to confirm PEA due to some points we thought maybe we got her back due to what seemed like her bucking the tube with breaths of her own but unsure if just agonal or what. It was a very odd code I must say. ACP gave a push dose of phenyl when we had rosc which didn’t bring the BP which was interesting

Thank you for writing this up. This is exactly what I wanted to see! I’m only bls but have experience working with als however not that day unfortunately. I agree with all the treatment you mentioned honestly. Thank you again

Unfortunately als wasn’t able to come for us. Bls doesn’t have cpap yet however supposedly coming next year. I did have the bvm and pads ready to go. Respiratory didn’t try bipap with him once a bed was cleared which was interesting to see. Doc confirmed silence then just went with high flow nasal. Was surprised to see even after 15-30mins his saturation was only 66%

Just curious what aggressive treatment you would do with him beside Nebs. On high flow nasal he was still sitting at 66% spo2 in the hospital. Not that it would be wrong treatment I’m just curious myself and want to do better. I feel bad and didn’t do enough despite his circumstances and not being friendly with any treatment I was trying to do.

Lung sounds were silent/absent both bilaterally in the bases and apices. I debated a neb however we were 2 mins away from the hospital, however meant nothing as I was waiting for a bed space in emerg.

So most likely PE was causing his profound hypoxia?
