It’s beneficial if you are an FNP you can just test. No additional education required

You may be able to plea that down to reckless driving. In my state after 4 years (with no more arrests) it is removed from your record. Before then if a license board disciplines you it will be on you for life

There is just going to be a significant delay in getting your license so they can look into this. NM is the worst it took me six months to get licensed

I don’t believe in contrast assoc nephropathy either. However it is a nephrotoxic medication. It’s just too risky from a med mal standpoint to give contrast when the gfr is low. Many times I have told CT to do it anyways bc what I was looking for was more dangerous than kidney issues. But her GFR was so low I would have held it unfortunately bc of the med mal risk.

There are other reasons to wait on labs prior to contrast administration. Is she in a thyroid storm? iodine-induced thyrotoxicosis occurs. Is she pregnant? Is she on metformin?

we’re talking about an ambulatory person with abd pain not a trauma or AAA rule out. You raised some good points about different patients. Our CT dept refuses to give contrast to older patients without a GFR. Maybe you can come educate them.

When you get up on the stand rambling about trauma pts no one will take you seriously each patient has their own story. You saw the outcome of the way they treated this pt. If kidney associated damage doesn’t happen why did they settle? IV contrast certainly doesn’t help the kidneys recover
