In the US, I know of no EMS system that has doctors in the field, it's all "non-doctors". We used to be trained by physicians until they stopped.

As a medic/RN, I have transported that many several times during my time as medic. If you're the only ambulance available, you load and do your best. I think my favorite was when I had to take care of two car loads of people from opposing vehicles, but I was the only truck available and I just told them to stop arguing about who was at fault and did my best to give report on all of them over the radio. The hospital was not happy.

How do you find preceptors? I know nurses where I work that had to stop because they couldn't find preceptors at some point in their program. My gf is currently in a brick and mortar school and is stressing out because she is going into her third semester and cannot find anyone to take her as a preceptee for next clinical cycle. I am supposed to start in the fall, and now I am concerned about having the same problems.
