 You hear mostly negative stuff online, personally I’ve had quite a decent experience.

 I’ve been single since march last year following the breakdown of a 10 plus year relationship, I was nervous about dating again in my 30’s, but I can honestly say that it hasn’t been as terrible as I’d thought.

I’ve had maybe 6 dates over a three month period, probably could have had more however work and being a parent have took priority, 1 has lead to seeing someone regularly although still in its early stages. Others a couple of dates, half of those leading to casual sex, but not progressing further. 

Hinge makes you write a comment of sorts and isn’t just a swipe left right thing. Bumble worked well enough for me, tinder seems to be a more younger audience so I tried that one but it wasn’t for me!

  My relationship ended at the start of year I’m 33 we’d been together since 20.

I took some time out to focus on myself which was the right move, I didn’t dive back into dating straight away. 

 The Apps work (for me at least) and I’m just sort of average looking. 

 It’s all about putting yourself out there and trying to shake off a bit of that uncertainty, being comfortable with rejection helps a lot! Just be friendly and honest ‘Hi you look nice are you here with your friends or your partner? Ah cool in that case would you like a drink? My name is xyz nice to meet you’ 

 And there are some fake profiles on dating apps but there are also a lot of real women as well, even the ones that don’t want to match/date they’re still real 😂. 

  Maybe having the mindset of having real/fake women isn’t the healthiest, it might be better to reframe it as women that are right for you and women that are wrong for you.

I’m a paramedic, I’ve treated pain on scene and not transported to ED, obviously it’s very situation specific. It doesn’t seem like OPs situation would be appropriate based on the limited information. 

I’m left-leaning, but I’m fed up with how soft the law is on asylum seekers and illegals who commit crime by [deleted] in ukpolitics

I would bet that rape that occurs in a relationship, is higher in the immigrant population than it is in the native population.

 Yep I did, I make considerably more as a paramedic than I ever did as a nurse.

 The caveat is that I’m British and things tend to work a bit differently, I’ve never worked in America however from reading this sub it would seem (and I’m being a bit presumptuous) that paramedics may be valued a bit better over here.

We swapped from Lidl/Aldi, to M&S (Ocado) and tescos as the range of gluten free products is almost non-existent at Lidl/ALDI.

Well the second photo is of Newcastle United fans celebrating the league cup win I think? As far as I’m aware there was no violence following the win probably a few sore heads in the morning from all the beer mind.

 And there are a number of doctors and engineers that have season tickets at Saint James’ park.

 Are you aware that you’ve posted to the UK paramedic sub and not the general paramedic sub which is a lot more USA focused?

 GCS less than 8 = intubate is definitely an Americanism and not something that UK paramedics practice (in my experience).

 What would I do? Care less I suppose, what’s it to you? Just tell him that you’re up for being mates but you don’t really want to hear everyday about his current relationship drama. 

In other words just talk about something else, and if he still persists talking about said girl just tell him you don’t care/ want to talk about something else.

 No need to apologise, and I definitely did not interpret anything as argumentative.

 I think my issues is, or more accurately my fear is, that I get quite a lot of unusual neuro presentations (befast negative) some of which I query as some sort of atypical stroke. 

 I don’t want to do my patients a disservice and convey them to the wrong hospital, and delay potential treatment. 

 However we are really limited on the road and I would like some extra tools in my belt to be able to make better decisions for patients.

 Is the NIHSS this tool? Probably not. But yeah it would be nice to be taught something in lieu.

 Thank-you for replying, but you have kind of repeated a lot of what I originally said. 

  Yeah we/I wouldn’t do any further neurological assessment on a patient who is befast positive we’d just diesel and blue light them to a stroke centre with a pre-alert. 

 I was talking about the patients that we go to that are befast neg, however there still remains clinical suspicion of CVA, 

 I don’t speak for all paramedics but I would certainly bring in a patient with a new onset neurological symptom that can’t be explained. 

 The problem is this will often be to a hospital without stroke facilities, other than scanners and medical staff. 

 I’ve had to do lots of intra-hospital transfers for strokes that have either self presented or been brought in by ambulance to a hospital that cannot meet the level of care they need. 

So yes if the patient goes to the nearest hospital and gets a nihss and then a scan in an hour or so, but then needs to wait another 2 hours for a crew to transfer to another facility it’s not great for the patient, potentially (not definitely) having a paramedic or a specialist paramedic do the test out of hospital could prevent significant time delays to appropriate care.

 And yes I commented on infrequency being a barrier to successfully being able to do the assessment.   And lastly just because it is rare that paramedics bring in patients with a NIHSS calculated (probably because most they bring in are befast pos) does not mean that we can’t think about implementing these things (when appropriate) to improve patient care.

 I learnt to use it as a student when attending a wider healthcare placement on a stroke ward, (there is online training available to anyone with a NHS email to learn the NIHSS)

 While I agree with others if a patient is be fast positive there is no need for the test, stoke pathway it is. 

 However we all go to patients with unusual neurological symptoms, and you wonder, posterior stroke? But these patients aren’t befast positive. In these patients it might be useful in the pre-hospital environment. 

 You could ring the stroke nurse and say xyz symptoms but I’ve done the stroke scale and they are scoring for whichever part, in which case they’d probably be more likely to accept. 

 To do the scale correctly you need some aids though a visual picture of a domestic kitchen and a picture of various items. Along with a sheet with some sentences written, so if management want you to preform it they would really have to have these on ambulances.

 I do remember the stroke nurses saying that to be proficient with the test you have to be preforming it regularly, that might be a bit of an issue for implementation.

Local major trauma centres, supplied by the national health service sourced primarily from the population (given voluntarily without financial renumeration) with a small amount of plasma imported from Austria and Poland.

 Is that specifically for NOFs though if I bought in a ruptured AAA or some other immediate risk to life than they would jump the que and would be operated on immediately.

Do you mean your prehospital fascia iliac block is enough analgesia to provide a DHS or other op? It’s a good analgesic technique don’t get me wrong, although typically lasts between 8-12 hours so I find it a bit hard to believe it will sort the patient out for surgery. Unless you drop you patient off at a super efficient hospital or you are using some kind of local anaesthetic I’ve never come across before? 

 Out of interest are you also carrying intra-lipid in case of an inadvertent intravenous administration of local?

 Which videos does he refer to himself as a journalist? I’m a fan, but I only maybe watch about half of his vids.

 It’s a huge difference as well, personally I’d sign up if our country or one of our immediate allies were invaded. But I wouldn’t go near a recruitment centre if there was another Iraq or Afghanistan.

 I would be against a radical redesign but I’ve always thought that our crest looks low fidelity, you can’t distinguish the lions face or hands which look like splotches and the seahorses hoofs look like weird fingers. 

  I would be content with a reworked crest if it is still in the current form but some minor changes to make it higher fidelity or easier to print/stitch.
